If you try going to that picture's URL, you will notice that static.tvtropes.org/pmwiki/pub/images/persona_4_3425.jpg shows the picture while static.tvtropes.org/pmwiki/pub/images/persona_4_3425.JPG returns a 404. Different capitalization = different file, thus the wiki can't fetch the image with the capitalized file extension because it doesn't exist.

As noted, it's not a bug. Media filenames are case-sensitive.

I thought this bug had been squashed many, many years ago, but I just saw an instance of it in the wild. Like a prehistoric coelacanth caught by a fishing trawler.
