This scene from Gettysburg, at the very end where General George Pickett, who up till that point has been excited and confident about carrying out the attack, realizes that the charge isn't working and all his men are getting slaughtered. The confusion and horror in his voice always twist my gut.

I feel posting a video of any of the scenes I feel to be most heartwrenching would be spoilers, so I'm just going to say- the ending to the Walking Dead Game. Actually, the entire fifth episode, really.
"It's so hard to be humble, knowing how great I am."Walt's death (spoilers) from Breaking Bad. Yeah, he wasn't a good guy by any stretch, and probably deserved this. But just something about the combination of his calmness and the upbeat song really gets me down. It's like he's accepted that his life has basically been a failure- that he's dying alone, unloved, and hated by everyone- and taking comfort in the last true friend he has. The implication that he's effectively lost his humanity and always cared about his meth, his true love, more than his family, is also quite sad.
Also, from a meta perspective... this was the end of the series. Another big reason for it to be heart wrenching.
