You won't need Origin if you have it on Steam unless you want to download patches (hint: You will.)
This thread has a few decent lists on the difference between TS2 and TS3.
The custom content I've seen is decent, but then I never found much TS 2 content I liked either. Just know that installing a bunch will increase load times a shit-ton, much more than in TS 2.

Just the addition of weight options makes a large difference (although, at least on my copy, there's a frequent glitch where people show up at their skinniest and only gain weight when you center the screen on them. One thing I do find a little annoying from a UI perspective is the difficulty of changing multiple outfits or hairstyles. You have to enter into the "Change Appearance" dialog (and wait a minute or so for it to load the data each time) for each outfit that you might want to change the hairstyle for.
You don't need to get the patches from Origin, you can patch it manually with the Super-Patcher.
Also, I don't like that you can make men huge and round or super-muscular but women barely change size on max settings. :/
So you can make them fatter, everything else looks the same, save maybe dying their hair.
