I think it does work, although I do have a slight misgiving in that the guys might be only paying attention to her because her costume sticks out like a sort thumb rather than because she's incredibly attractive. Not sure whether that distracts from the illustration, though.

I'm in favor of 49, if only so that there's no confusion with the aforementioned Hospital Hottie. While it's true that the men could be reacting more to the conspicuous costume, I think it's clear enough that they're reacting to her general appearance.
