Maker's Gift
In ages past, a great warrior accomplished eight great deeds in service to the God of Machines*. As a reward, the Great Maker forged a weapon - the blade Maker's Gift.
This weapon resembles a large double-edged sword of black iron, save that the edges of the blade are forged into the blocky teeth of a gear rather than a smooth line. When wielded, a mystical field of near-invisible energy* envelops the blade and provides a supernaturally keen edge.
The guard bears that ancient warrior's personal heraldry embossed in gold upon a cogwheel, and its arms are shaped like intricate mechanical wings. A large ruby, its shape reflecting the cogwheel motif, is set into the pommel.
My players once got hold of a box that contained a tiny portal to the Elemental Chaos. Every time it was opened, the players would have to throw a 1d8 for all kinds of effects, including very localized earthquakes, floods and tornadoes.
They managed to use the box as a hand grenade, claymore (the explosive, not the sword), Shmuck Bait, distraction and intimidation device.
It was quite an awesome box.

We once used a portable hole as a private aquarium. There was a dolphin in there.
And not just any dolphin, it was a celestial dolphin.
EDIT: Same campaign, we also crafted gauntlet-crossbows that launched Dearn's Instant Fortresses at people.

This has been going on for years and nothing seems to turn his luck around (He's a good sport though, keeps playing). In one game he was playing a magic users, and when he'd roll a 1, players would say "He dropped is magic!" (After the idea that regularly, fighters who critically miss drop their weapons). Thus was born....
The Magic-Holding Strap.
Allows you to re-roll a 1 on an attack roll for an arcane power. It straps one's magic in place, making it harder for one to drop their magic.

Yeah, Dearn's Instant Fortress is a tower that collapses into (and expands from) a cube that's an inch or so wide. If it lands on someone, it does 10d10 damage (might be 10d6 actually). Naturally, we weaponised it.
Mk Iv Polyphemus optical implant
It's an eye-laser. This implant replaces a character' left eye, and grants them darkvision as well as a ranged natural weapon that does not provoke attacks of opportunity.

Precept is simple. Here, post awesome rewards to give to players. Anything is on the table- stone age to future tech, with as much magic as desired. Even non-item "loot" is cool- the blessings of a magic spring, or a deal with a/the devil, for instance. Assume you're not constricted by system, only by awesome. It'd also be cool if people could give requests- e.g. "I need stuff to put on a sky pirate airship"- and have people throw some stuff at them. Be as detailed as you like, also- anything from a blank slate with just a set of abilities to a fully detailed item with its own history.
