Also, another possibility, he could be somewhat sensory-deprived and he relies on his intuition/the voice of his sword to function.

So, rather than listening to the subtle hints as to his environment that his intuition gives him, his sword guides him. But really, it's just his mind, giving him directions as per his intuition?

Y'know how samurai believed that their swords were sacred, had a spiritual essence/soul and sentience? (Holy alliteration, Batman!)
I was thinking of a concept wherein it seems like the sword is sentient (ie, has conversations with its owner) but in fact the samurai in question is just a wee bit loopy and imagining the sword's intelligence.
