It's a boy and a girl kissing in Hell. * What more do you want?
We could use the "not a prison anymore" bit from Order Of The Stick, but that might be a bit of a spoiler. This one's obscure enough to not be, I think.

Well, I honestly couldn't tell what was going on in the background. I just knew there was fire. They could have been kissing after calmly exiting a house that was burning down or something.
Infinite Tree: an experimental storyYeah, but even if it was a house fire, it still looks like Hell, so I think the image would work even then.
I couldn't tell it was a fire. It kinda looks like a bright sunlit sky with a cloud behind the girl's head.
Even if they were in hell, how do you know that one of them chose to be there because of the other? They could be just any two residents.

Bad, bad picture.
^^^But it, when combined with the title, merely made me think the trope was "I've metaphorically been through hell to get back to my lover."

Infinite Tree: an experimental storyWell I figured the fact that they were kissing instead of screaming in torment was what brought it across.

But nothing about it or the title said "literal hell" rather than "going through hell" in the sense of "facing difficult challenges" to me.

Infinite Tree: an experimental storyI would be ok with a larger version of the pic (one that's not cropped to just their faces, I mean), so it's more obvious that they are in hell, but I don't think it will be possible to get across that one of them consciously chose to be in hell so they could be with the other in one picture.

All the shots I could find are of just their heads, unfortunately, and I don't have a DVD to cap from.

Well, then, we need to find another source for the image.
Infinite Tree: an experimental storyI think the movie (it's Return Of The Living Dead 3) had a shot of them lying down together in the flames, but I can't quite remember and like I said, I got no DVD.
Perhaps someone who does could help out.

Has anyone looked at the Sinfest example on the page?
The one that's linked is sweet but not completely clear with out knowing the characters, But this one◊... I think it works.
...if you don’t love you’re dead, and if you do, they’ll kill you for it.Between her being a devil and the cave-like BG (oh, yeah, and the mention of "down here"), would panels 2 and 3 of the above suggested strip work?

That's what I was thinking.
...if you don’t love you’re dead, and if you do, they’ll kill you for it.That sounds like a plan.

Becky: Who are you? The Mysterious Stranger: An angel. Huck: What's your name? The Mysterious Stranger: Satan. That works.
Reality is that, which when you stop believing in it, doesn't go away. -Philip K. DickI dunno, I like having her reaction shot at the end, and I don't think it would hurt to have the three panels stacked on each other...I'll play with it and see. Also, with the two-panel version, while it's obvious that they're in a cave-like place, it could just be a cave. In the three-panel version, there's flames behind them, which brings in the "hell" aspect of the trope.
EDIT: Here we go.

The description is long enough that the three-panel vertical should drop too far into the examples. if it drops into them at all. I can deal with that.
...if you don’t love you’re dead, and if you do, they’ll kill you for it.I like that. The article is maybe not long enough to keep the image from poking past the hr into the examples (especially on a widescreen monitor), but, I get the impression that I care about that more than most people :P What about a 2x2 of all frames, though?
Becky: Who are you? The Mysterious Stranger: An angel. Huck: What's your name? The Mysterious Stranger: Satan.

Nominations for replacement images:
The current picture does not show the trope, I have no idea what's going on other than two people kissing.
