The Kaffarov mission also has the Barret .50 cal equipped with a IRNV scope, though for some reason it's unnamed. You'll find it at the beginning of the underground shooting range on the load out table.

^ Actually if you simply wait long enough at the door before you beat down the guy with the SV-98, it simply appears in your hands out of Hammerspace.

Necro time: I'm stuck on Operation Guillotine on PC due to a seemingly impossible QTE, namely the one with the PLR soldier in the washroom, as the instant I hit "E" the game immediately tells me I failed the (left mouse button?) cue afterwards. Anyone know what my exact problem is?

Damn, sorry I can't say that I do. You've got no choice but to do the unenviable task of digging through the BF 3 Battlelog forums and figuring out if there's a patch available for the bug you're experiencing... not fun, especially given how brain-dead most of the posters on those forums are.

BTW, there's a much worse version of the glitch you're talking about at the very end of the Co-Op missions. It involves the fact that you and your buddy are supposed to synchronize your QTE button presses, and that DICE apparently forgot that some players internet connections are not that reliable... meaning you're gonna be stuck repeating the entire mission over and over again until your internet decides not to be a bitch and screw everything up for the both of you.

I figured out what happened: When the game wanted me to press a particular key, it wanted me to do so once unless otherwise instructed. Brilliant.

Now that I've dealt with the single player mode, now for multiplayer... and my perennial problem with stats/unlocks not registering.
