  • Fan Nickname: The section of Stage 6 before the midboss appears is infamously known in the fanbase as "Spam", due to the sheer volume of danmaku that can't always be dodged reliably, especially on Lunatic, and even more so if you're playing a forward concentration shot type like Reimu-B or Marisa-A. This kicked off the trend of other dense and overwhelming stage sections being similarly dubbed "Spam", but this one continues to be considered the hardest and most infamous of all, and when fans say "Spam" without specifying the game, they usually refer to this one.
  • Game-Breaker: Sakuya's shot types. Firstly, she gets four bombs per life, instead of Reimu's three. Her A-type shot is a giant spread when she's unfocused, and a single-target homing shot when focused, with bombs that share the same properties. Her B-type shot, once mastered, essentially has the coverage of her A-type shot with the power of ReimuB, as well as lockable options and a bomb with a very long invincibility frame. The only downsides are Sakuya's fast focused movement speed and Merlin's opener.
  • Goddamned Boss: Ran as the midboss of the Phantasm Stage. She's not exceptionally difficult, and in fact might even be a bit easier than her Extra Stage counterpart Chen, but she's also the only midboss ever in the series to have a bomb shield, which can make her annoying to deal with if you thought you'd be able to fall back on a bomb just in case.
  • Growing the Beard: While Embodiment of Scarlet Devil introduced the basic fundamentals of what would become the Windows-era of the franchise, this game is generally held as the point where the series identity really solidified itself, introducing or refining mechanics that would become the baseline standard of the series going forth. In addition, it is also the game where the setting of Gensokyo becomes fully defined, in particular with the introduction of the Great Hakurei Barrier and Yukari Yakumo.
  • Hilarious in Hindsight: Youmu has a Spell Card called "Animal Realm Sword". Ten main series games later, the Animal Realm is properly introduced to the series, and Youmu is playable in that game to boot.
  • Low-Tier Letdown: Reimu-A. She's very weak even compared to other games' homing Reimu shot types, and her bomb isn't very powerful either. Adding to this is the fact this game offers a stronger homing shot type in the form of Sakuya-A.
  • Sequel Difficulty Drop: This game adds some convenience features that make it easier than its predecessor, with visible hitboxes when in focus mode and the Supernatural Border system that creates a Single-Use Shield.
