I honestly wonder if the writers intend that. She sees dead people. At first it seems it's for the audience's benefit, a representation of her thinking about what they might say or do. But then she answers her mental Mr. Kaplan aloud and another character is confused. It seems she ''truly'' sees and hears Mr. Kaplan, and thus it was probably the same with Tom when she similarly talked to him long ago. Many actual schizophrenics know hallucinations when they see them and simply ignore them. "That penguin dancing on the ceiling in a clown suit probably isn't real because penguins who dance on ceilings in clown suits aren't a thing. My brain's being weird again; on with life." Of course, her biggest hallucination is at the end of season 8. Yes, she ''is'' seeing all of that the way we do.
