• Missing Episode: The Most Wanted series of villain roster books went straight from #1 to #3 over rights issues.
• Sequel Gap: The adventure "Search for the Sensei" announced a sequel was in the works, "Revenge of the Yakuza". The former came out in 1986, the latter didn't come out until 2017. The manuscript got lost in the mail when the company relocated its offices to another state, and it wasn't until many years later they made contact with the writer again, who found a spare copy they could use for publication.
