As expected from a fanfic about A Certain Magical Index, expect some heartbreak to go along with the laughs about Touma's misfortune.

  • During the Misaka Mikoto Arc, Misaka does research about Touma to learn about any weaknesses he might have and instead learns of his Dark and Troubled Past, how people would get hurt around his misfortune to the point that he was known in his hometown as the God of Plague, how he has a scar on his back from where a man tried to stab Touma to death because him interacting with Touma caused his company to fail and go bankrupt, and how to TV studio wanted to do a Reality TV Show based on his misfortune just for some ratings. Misaka is understandably sickened by this and resolves to keep the promise she made about helping carry his burdens.
  • In the Washing Machine SS, All of the nuns in Necessarius along with Birdway and Lessar deconstruct Touma's Chronic Hero Syndrome and his taking blame for the events in London during the Orsola Arquinas arc to the Thor arc. It devastates them to know that he undervalues himself so much that he considers their smiles to have more worth than his own life. Not only that, but it shows that he's actually aware of people both on the Science and Magic sides to want to help him and yet still refuses to say the most precious words they want to hear: "Please help me". It doubles as a Heartwarming Moment, as it solidifies their resolve to be part of the "Kamijou Faction", to have his back and support him even if he refuses their help.
  • In Uiharu Kazari 22, the titular character of the arc can only watch in abject horror as her friends are at each other's throats over the decision whether or not to warn Touma of Thor's arrival. While it is a Foregone Conclusion that they don't, the fact that Kuroko and Misaka actually screaming each other due to Thor's warning that if they warned Touma, Thor would kill him is heart-wrenching. The fact that Fire-Forged Friends such as them almost fell apart due to their feelings for Touma is a bitter pill for them to swallow.
  • When Uiharu knocks out the MINUS General, who was inside the bus, with a wrench (to the point that blood is oozing from their head), Saten then asks her about the guy she knocked out. After finding out the identity of the person she briefly screams, and lashes out at Uiharu. Uiharu is both confused and horrified as to why Saten was angry at her (the reason why Saten was pissed off was because Touma was the guy that Uiharu knocked out at the beginning of Chapter 41 due to her erroneously thinking that he was The Leader of MINUS.); after finding out what she did, she didn't take it very well.
    • One thing to note was Saten's reaction to her own actions towards Uiharu. After finding out that Uiharu didn't realize that she knocked out Touma with a wrench, she immediately regrets it.
  • Touma goes through a momentary Heroic BSoD when he realizes that his actions during the Sisters Arc of the canon series resulted in the creation of MINUS; when all he was doing was trying to save the smiles of Mikoto and her remaining 9,969 clones.
  • Uiharu's Love Epiphany is actually heartbreaking rather than heartwarming. The reason behind it is that Uiharu made an image in her mind of Touma being a Knight in Shining Armor, but the fact is that Touma's actions and mentality made that said image of the knight lacking, compared to the reality that she faced. Not only that but considering that her closest friends are also in love with him makes it a twisted sense of feeling closer to her friends but at the same time feeling as though she betrayed them.
  • Touma realizes he has PTSD from his ordeals at NT 9, at the epilogue of the last chapter of the Uiharu Kazari arc. Just goes to show how mentally broken he is.
  • There are two main reasons that Touma believes that a romantic relationship with him would end horribly in Kongou's arc, and it is heartbreaking. His memory loss and his misfortune:
  • What makes things worse is that we see this happen live in Chapter 151. Just when you thought that the fake date is salvageable "WHAM" his PTSD hits him hard and Touma had a huge panic attack in the last part of the date. This ends up humiliating Kongou to the point of ending everything, cursing Touma and walking out off a broken Touma.
