* SecretRelationship: Multiple occur throughout the series, sometimes lasting for quite some time:
** Levin and Lyuba developed a relationship in the first season, but chose to keep it hidden to preserve their dignity (slim nerd and chubby nurse; they're doomed to be a laughing stock). Levin accidentally reveals it by himself when he thinks that everyone knows already (and mocks them) and tries to defend Lyuba. Ultimately, everyone reacts just fine, and the two continues dating openly.
** Bykov and Kisegach started dating each other early into the series (and were "friends with benefits" long before then), but tried to hide it from everyone at first, out of fear that Gleb (Anastasia's son) would find out, since he and Bykov can't stand each other. Gleb eventually finds out anyway, but learns to accept it, and need to keep in secret died out by itself. Eventually, the two would marry and have a son.
** Kupitman and Lyuba started dating in late seasons, but chose to keep it secret out of fear that everyone would mock them. When it stopped being secret after Kupitman had a near fatal heart attack and Lyuba had to explain how she, Kupitman and viagra are related... turns out that ''no one'' (excluding Lyosha, who previously was forced into role of SecretKeeper, and even that is likely due to ''how'' he learned about it) thinks that there's something worth laughing about, and they quickly calms down. In the final episode, Kupitman even proposes to Lyuba.

* EverybodySmokes: Romanenko and Lobanov smokes quite often. Kupitman and Kisegach are occasional smokers (the latter usually does it to deal with stress). A lot of secondary characters smoke as well. The hospital even has a dedicated smoking room, despite smoking in hospitals being banned in real life.

** In season 4–5, Bykov and Kisegach have a brief fallout, during which, Kisegach started dating Anton Yaroslavsky. Eventually, she dumped him on her own, and returned to Bykov.
** In season 5, Semyon (who by that point already divorced Olga) starts dating Irina; but in season 6, Olga returns and starts dating Phil. But Lobanov now tears apart between OldFlame and new love.
** In season 7, Semyon starts trying to make Olga break with Phil and return to him. While eventually she does break with Phil, it takes Semyon some more time to make her go back.

* PerpetualPoverty: The hospital itself is always underfunded, forcing Kisegach to [[CuttingCorners cut corners]], or bribe inspectors. It mostly just gets mentioned in background (mainly as a reason why one of the doctors is angry at her), but two episodes involves Kisegach's attempt to save money on ''fire safety'', of all things.

** Bykov and Kisegach often interact with each other using some sorts of cryptic messages, for whatever reasons. Because of this habit, when something ''not'' caused by one of them happens (like anonymous gifts, or ''important visit from ministry''), one of them would suspect this to be some riddle from other one, and either would reply in turn (starting ''long'' play which only ends when one of them decides to confront other one directly), or would spend whole episode hiding (despite theres something ''really'' important happening which requires their presence). This mainly happens with [[ManChild Bykov]] (especially second variant), but Kisegach is not immune either.

* PunishmentDetail: Most common punishment for interns is to stay on a night shift. It's frustratingly ''boring'', and you would be sleepy next day (not to mention you would be forced to cancel any plans you have for this day). Bykov also can force you to do orderlies' job, like cleaning duty – in front of your colleagues and patients. Of course, it gradually lost its effectiveness due to how commonly it gets issued, especially on Lobanov.

** Phil (who has two gay fathers); every time his parents or other relatives are involved, expect some jokes at expense of homosexuals. Phil being [[CampStraight "not truly straight"]] gets mocked occasionally, too.

** Almost every time when Lobanov has to pay for something, [[PerpetualPoverty he would attempt to loan money from somebody]] (even before his divorce with Olga).
** Every time Kupitman calls Bykov on something, Bykov would tell him that he looks like "drunkard granny".
** Levin and anything that has to do with [[AnimalMotifs deers]]: hangers, [[HomemadeSweaterFromHell sweaters]], statues of actual deers... And Lobanov is always here to mock him for it (in fact, he is the one who started all this, by calling him "[[MooseAreIdiots deer]]" after Levin's AlcoholInducedIdiocy).

** Bykov (like his actor) [[SpeechImpediment can't pronounce letter "R"]]. This is milked for all its worth, sometimes even by Bykov himself.

** Bykov and Kupitman. They are best friends for a ''long'' time, and always stick together (even if they [[{{Troll}} can't stop trolling each other]]). In the end of series, when Bykov decided to move to Saint-Petersburg, Kupitman decided to go with him, because he can't stand being left alone.
** Semyon and Phil, of all people. While initially he openly bullied Phil, later Lobanov considered him to be one of his friends, even when Phil started dating Olga (Lobanov's ex-wife to whom he still has feelings). While initially opposed (he even hit Phil in the face), he later let him date her anyway, [[IWantMyBelovedToBeHappy because he and Olga genuinely loves each other]]; conversely, when Gleb [[KickTheDog scammed Phil for a big sum of money and bragged about it to Semyon]], Lobanov [[WhatTheHellHero called him a vermin for mistreating his friends]], and forced him to return money and apologise. They still make fun of each other, but it is just friendly trolling, to make life funnier.

*** In first season (where their relationship started), they briefly broke with each other; it lasts for two episodes, with only one actually being about them, after which they reconciles.
*** In the last season, much later after their permanent breakup and Levin moving to USA, Levin [[BackForTheFinale returns to work in the hospital]], even more arrogant than before... and Lyuba is [[WomanScorned still bitter]] after their last meeting. While they're now working in different departments, whenever they meets, it shows that they're unlikely to ever come along again. The true problem comes from [[ImaginaryLoveTriangle Kupitman believing that Levin is trying to steal Lyuba from him]].
** After her divorce with Lobanov in the season 4, Olga returns in the season 6 to work in the hospital, as an accountant. Fortunately, by this point she and Semyon are no longer on bad terms. Unfortunately, Semyon is now in different relationship, with his new girlfriend being jealous, while Olga quickly gets a new suitor -- Phil. The two eventually reunites.
** Bykov and Kisegach, briefly. When he cheated on her, their relationship was ruined. What makes this particularly bad for Bykov is that she is not only his lover, but also his ''boss''. Fortunately, she decides to give him another chance, and now they are married and have a young son.

** Phil; every time his parents or other relatives are involved, expect some jokes at expense of homosexuals. Phil being [[CampStraight "not truly straight"]] gets mocked occasionally, too.

** Phil; every time his [[HasTwoDaddies parents]] or other relatives are involved, expect some jokes at expense of homosexuals. Phil being [[CampStraight "not truly straight"]] gets mocked occasionally, too.

* AnnoyingPatient: If the problems aren't caused by interns of Bykov's weird antics, they would often be caused by the patients themselves. Some are dumb and/or arrogant, some are rich and entitled, some are outright insane.
* ComicallyIneptHealing: All over the place where interns are concerned; unfortunately, [[TakeOurWordForIt most of it happens off-screen, with only Bykov's reaction being shown]], but some cases which we actually learn about shows just ''how'' bad it can be:
** In [[EstablishingCharacterMoment his very first episode]], [[TheDitz Semyon]] (in direct violation of Bykov's orders) blindly injects his (old and frail) patient with a medicine he only vaguely remembers as the right one. By sheer miracle it turns out out to be correct, but it clearly shows [[LethallyStupid his approach to his job]].
** [[SmallNameBigEgo Levin]] receives a patient with vegetative-vascular dystonia (pretty common condition about which viewers were informed upfront, because said patient is [[AnnoyingPatient Bykov's old headache]]); instead of correctly identify it, he came to conclusion that it must be some new, unique illness he can research and name. So, he proceeds with countless senseless tests, culminating in attempt to take a sample of the ''patient's brain''.

** Bykov, Kupitman and Kisegach always addresses interns by their surnames (Bykov can also use nicknames); [[FirstNameBasis exceptions]] are Varya (only Bykov calls her by surname) and Phil (''only'' gets called by first name).
** Most patients, if they gets named at all, are referred by their surnames.

** In the season 14, Victor Romanenko returns to get Anastasia back; with Bykov acting like [[EntitledBastard his usual self]], he almost succeeds without even trying much, and Bykov saves his marriage at nearly the last moment.

* PoorCommunicationKills: Frequently, the plot gets started by characters being unable to properly communicate. Recurring offenders are:

** If the patients gets named at all, they would be addressed by last names, with few exceptions.

* PerpetualPoverty: The hospital itself is always underfunded, forcing Kisegach to [[CuttingCorners cut corners]], or bribe inspectors. It mostly just gets mentioned in background (mainly as a reason why one of the doctors is angry at her), but two episodes dealt with Kisegach's attempt to save money on ''fire safety'', of all things.

** Varya either can't explain what she actually needs from her patients (this is when [[ExtremeDoormat they actually abide her orders at all]]) or other people, or keeps withdrawing crucial information for no coherent reason.

* PuttingTheMedicInComedic: The medical sitcom about the life of a "common" Russian hospital and its inhabitants. Events may range from just funny to downright surreal.

*** In the last season, much later after their permanent breakup and Levin moving to USA, Levin [[BackForTheFinale returns to work in the hospital]], even more arrogant than before... and Lyuba is [[WomanScorned still bitter]] after their last meeting. * TheAlcoholic:
** Venereologist Kupitman, big time. He pretty much runs on Cognac, and has personal collection of it. He also uses it as PracticalCurrency. In fact, his [[EstablishingCharacterMoment his very first appearance]] was when he arrived on the work with heavy hangover and asked Bykov for booze to "cure" his condition.
** Electrician Ivanovich. That's a miracle he still lives (let alone keeps his work), despite often working while being wasted.
* AnimalMotifs:
** Levin and anything related to (or resembling) [[MooseAreIdiots deers]] (hangers, [[HomemadeSweaterFromHell sweaters]], statues...) Started as random insult from Lobanov after [[AlcoholInducedIdiocy particularly serious screwup]], but quickly become RunningGag.
** Kupitman and poodles (probably because of his hairstyle). Started from lame insult from Levin whom he screwed over ([[ManipulativeBastard again]]), but sometimes gets remembered by other characters.
** Bykov and lizards (taken from his old biker nickname).
* BadassBiker: Bykov used to be one under nickname "Lizard".
** In one episode he met his old friend Boar, and almost decided to quit here and now... only for Boar to remind him what he (and many others) needs not Lizard, but doctor Bykov, and it would be selfish to do it.
** Much later he decided what he may be ''both'' Lizard and Doctor Bykov, and bough himself new bike. This lasted for several episodes... until he was hit by the truck and nearly died.
* TheBully:
** Lobanov, mostly in regards to Levin, though he also briefly abused Phil.
*** Semyon loved to play pranks on Levin. He was also one to start calling him [[AnimalMotifs "deer"]].
*** in Phil's case, one whole episode was dedicated to Lobanov trying to impose "Dedovshina" on Phil (basically, "I work here longer than you, ergo, I'm superior; now, bring me coffee and clean my boots" mentality common in Russian army, often combined with physical abuse). This was quickly stopped by Bykov, who [[HoistByHisOwnPetard used same logic against]] ''[[HoistByHisOwnPetard him]]'': after all, Bykov works here even longer.
** Gleb also mistreated Phil, but in more subtle way, mostly by exploiting his [[NaiveNewcomer naiveness]]. This is when Lobanov already started considering Phil to be his friend.
** Speaking about Gleb, he used to be one back in high school, cruelly abusing his fat schoolmate; it come to bite him in the ass in one of the episodes.

** Bykov may act like EntitledBastard and utter {{Troll}}, but he ''would'' help you if you really need it (even if he would mask it as insult, which never fools anyone). Specifically, each intern have seen his soft side at least once:
*** When Lobanov's marriage was in danger, only Bykov tried to actually save it, while Semyon acted like careless moron (which in the end doomed any chances he had).
*** When Romanenko, to ''everyone's'' surprise decided to stay in the hospital, only Bykov actually congratulated him.
*** When Chernous [[PutOnABus left the therapy department]] (to work in surgery with her new LoveInterest), at first she considered Bykov's happy reaction meaning him not giving a crap about her. Instead, he was happy what she, first of his students, found her place in life and became independent from him. They parted on good terms.
*** Alexei quickly earned Bykov's wrath for his incompetence, and Bykov gave him an ultimatum: either Lyosha treats at least one patient, or Bykov would kick him out. Then Bykov learned what Lyosha is good with broken tech (showing the actual analytical skills in process), and forced him to apply his skills to medicine (by using technical terms as metaphors for human body). Alexei still failed in the end, but Bykov realised what he ''has'' potential, only missing self-confidence, and not only allowed him to stay, but personally showed what exactly he did wrong.
*** When Bykov learned what Maxim faked his diploma (because he was kicked out from the medical academy, which he tries to hide not so much to cheat his way into medicine, as because he fears to become [[BlackSheep a disappointment for his family]], Bykov agreed to keep it secret on condition what Max would quietly quit himself and finish his education; he even promised to accept him back if he ever decides to return.
*** Despite having uneasy relationship with Levin, Bykov ''did'' consider him to be good new Head of Therapy and Bykov's replacement -- "[[DrillSergeantNasty because there's always must be someone whom everyone would hate]]".
** Lobanov, while being [[HairTriggerTemper explosive]] and [[{{Troll}} loving teasing other people around him]], ''would'' protect people he considers to be his friends (including from other friends). He also often becomes that one guy who [[WhatTheHellHero calls other people out]] on ''really'' [[{{Jerkass}} going too far]]; not even Gleb, his best friend, is immune to this.

** Besides Lyuba (before their breakup), nearly no one calls Levin "Boris".
** Bykov always addresses his interns and doctors by their surnames (unless he uses nicknames).
** Bykov, Kupitman and Kisegach are mostly called by their surnames instead of first names, even when they talk to each other.
* TheNicknamer: Bykov has humiliating nicknames for any intern, but special mentions deserves these ones:
** He calls Varya "Meine lieben Püppchen" ("My beloved dolly" in German).
** He refers to Phil as "Filya" or "Fil'ka", the Russian version of the name, and ShoutOut to [[https://en.wikipedia.org/wiki/Good_Night,_Little_Ones! popular kids show]] with [[FunnyAnimal puppet dog with same name]].
** Levin constantly gets renamed to "Doctor Lenin", [[MaliciousMisnaming despite his protests]].
** Maxim is "[[https://en.wikipedia.org/wiki/Korney_Chukovsky Chukovsky]]", obvious joke at his surname, Korneev.
** Gleb gets the worst of it: "Mishka kala" ("Bear of excrements", a play on "Mishka koala", "Koala bear").
* PerpetualPoverty:
** Lobanov is [[RunningGag always]] low on money, and needs to loan from somebody. Interestingly, this started ''before'' he got divorced with Olga.
** The hospital itself is always underfunded, forcing Kisegach to [[CuttingCorners cut corners]], or bribe inspectors. It mostly just mentioned in background (mainly as reason why one of doctors are angry at her), but two episodes dealt with Kisegach's attempt to save money on ''fire safety'', of all things.

Unfortunately for him, these schemes [[SmugSnake rarely works out]]. Also, he considers himself to be superior to pretty much everyone around him, especially Alexei, whom he mocks for his "lower-class" upbringing.
** Kupitman, to the entire hospital. He is [[GreedyJew greedy]], he is [[ItsAllAboutMe selfish]] and he [[ChronicBackstabbingDisorder sees nothing wrong with backstabbing his friends and colleagues if it means some gains]]. And then his sick sense of humour comes in; he likes playing other people for his sick amusement.
* TokenGoodTeammate:
** For first team of interns, we have Varya and Phil (and Varya later [[PutOnABus left]]), who works with likes of [[SpoiledBrat Gleb]], [[HairTriggerTemper Semyon]] and [[InsufferableGenius Levin]]:
*** Varya is most emphatic one around here (often to her own detriment), and always ready to help people in need.
*** Phil is resident NiceGuy, always polite and friendly ([[BewareTheNiceOnes but even he has his limits]]), and seems to be only one to not tolerate rules-breaking (even including administration). This makes him working under [[TokenEvilTeammate Kupitman, who is almost his opposite]], all the more ironic.
** Alexei is nice, friendly and cheerful, and always ready to help if he can. He works with [[InnocentlyInsensitive Polina]], [[IceQueen Sophia]] and [[ManipulativeBastard Maxim]].
* {{Troll}}: Characters just love trolling each other, on- and off-duty. Amongst them special mention deserves:
** Bykov. He trolls ''everyone'': his interns, his friends, his LoveInterest and his patients. [[BitingTheHandHumour He would also troll his boss's sponsors]], had she allowed him to contact with them in any way.
** Kupitman. He is more subtle than Bykov, and mostly uses him as target rather than anyone else. This is some sort of competition between them: who would be first to play prank on another one.
** Gleb. Unlike Bykov, who does it [[TheGadfly just for fun]], or Kupitman who usually just likes to play with his friend, Gleb's pranks are more malevolent and harmful to other characters.
** Maxim just likes mocking Alexei, whom he considers "[[WorkingClassPeopleAreMorons inferior]]" tom himself.
* PuttingTheMedicInComedic: Medical sitcom about life of a "common" Russian hospital and its inhabitants. Events may range from just funny to downright surreal.
* TokenEvilTeammate:
** Kupitman is greedy, selfish and tend to manipulate people either for his own gain or just sick amusement. Because of this, barely anyone besides Bykov and Phil (who's his subordinate since the end of internship) sticks with him.
** Gleb is [[{{Troll}} cruel troll and prankster]], who likes to manipulate other people into doing his work for him, and has rather louse moral standards.
** Maxim is haughty, selfish and has rather lousy moral. He also can't stop abusing Alexei, closest he has to a friend.

** Recurring joke in the series is somebody (mostly Lobanov, which is especially ironic considering he is quite homophobic) being MistakenForGay because of something [[NotWhatItLooksLike seen]] or [[OutOfContextEavesdropping heard]] out of context.
** One episode has Kupitman's [[MissingStepsPlan "genius" plan]] to [[CureYourGays seduce woman]] by pretending to be CampGay. With Bykov as his "partner". It went as well as may be expected.

** Every time Kupitman calls Bykov on something, he [[NoYou would tell him what he looks like "drunkard granny"]].
** Levin and anything what has to do with [[AnimalMotifs deers]]: hangers, [[HomemadeSweaterFromHell sweaters]], statues of actual deers... ** Kupitman and [[AnimalMotifs poodles]]; whenever opportunity arises to mock his similarity to poodle, it wouldn't be missed.
** Bykov (like his actor) [[SpeechImpediment can't pronounce letter "R"]]. This is mocked to all its worth, sometimes even by Bykov himself.
** Alexei just can't shut up about Torzhok. When it became particularly annoying, [[RunningGagged Bykov put a fine system to discourage him]].
* TheSlacker: Gleb and Semyon. They would do anything to avoid doing work.
** Lobanov often creates elaborate schemes to just leave early. They almost never work, either because of genuinely terrible plan or just as terrible luck; in fact, more than once [[ImStandingRightHere Bykov just overheard everything right in the beginning]].
** Romanenko, on the other hand, would try to fool or blackmail other people into doing his work for him, mainly when he is assigned to work with somebody. This often backfires to the point he must rush everything by himself to avoid failing the task.
* ThoseTwoGuys:
** Gleb and Semyon. They always stick together, even past their internship, and often cooperate for either chicks, pranks, money or to [[TheSlacker avoid doing work]].
** Orderlies Pasha and Petr almost always appear together, [[LivingProp usually on background]].
** Maxim and Alexei are nearly always stick together, but unlike Gleb and Semyon, calling them "friends" would be far stretching, considering abuse Max puts Alexei through.

** Semyon and Phil, of all people. ** Varvara Chernous [[TheIngenue naive]], [[ExtremeDoormat weak-willed]] and [[ATragedyOfImpulsiveness impulsive]], which constantly interferes with her actual duties, especially when patients are not cooperative.
** [[SixthRanger Phil Richards]] is slightly better than other original interns, but due to being [[FunnyForeigner foreign to Russia]] suffers from either poor communication with his patients (and not because of language barrier), or from total lack of understanding of Russian mentality, customs and unfamiliar to him methods of "folk medicine" (while indeed questionably effective, it would be good idea to at least learn what to expect from local people), which often interferes with his work.
