In a unique twist, J.D. turns his narrating duties over to Ted, Todd and Jordan — as this episode focuses on the lives of the supporting players at Sacred Heart Hospital. Ted juggles fantasies in which he has a full head of hair and a reality where he is helping the nursing staff fight for a raise; Todd meets his future son and helps Turk preserve his reputation and Jordan puts aside her knack for causing misery to help Elliot and Keith's relationship.

- The Greatest Story Never Told: Ted helps the nurses by anonymously giving them an invoice that reveals Kelso has been paying $50,000 worth of needles for only $50. They use it as blackmail against him for their raise, and he denies being the one who was responsible for the leak.
- Ironic Echo: Kelso asks Ted if he would kill him, and laughs it off saying he doesn't have the guts. When Kelso discovers the nurses know about the needles, he asks Ted if he was responisble, and he replies that he don't have the guts.
- Large Ham: Ted with hair screams to the sky after accidentally killing his mother in his Imagine Spot.
- Lower-Deck Episode: Ted, Todd, and Jordan are all recurring characters who are not part of the main cast.
- Noodle Implements: For once, we get to see J.D. doing his Imagine Spot from an outside POV, and when he's done he mentions it involves a lot of gnomes.
- Wrong Assumption: When Todd's next patient is a 16 year girl set to get breast implants, Turk tries to stop it due to thinking she's being pressured to by her mother, even though it could be bad for his career. Todd simply takes Turk to the patient and states he's wrong: she's doing it because she doesn't look like her age and she shouldn't have to convince anyone else like her mother.
