It's the end of their 2nd year and everything starts to feel boring, but they have their college pal to cheer them up, and get them into trouble. Dr. Kelso is on his annual "picking somebody to pick on so that everybody will be scared of him", he picks up Elliot for almost blinding him, almost gets her to quit. Perry discovers that he is the father of Jordan's child, he gets angry and tries to break up with Jordan but she won't let it happen ,then he tells JD that he is not mad at Jordan but he is actually scared from the idea of being a father.

Ryan Reynolds guest stars as the old college friend of JD and Turk, Spence.


  • Big Eater: An Imagine Spot has JD as the Chocolate King, eating everything in his office, including his nameplate and Turk's hand.
  • Butt-Monkey: Ted has a computer keyboard, but no computer.
  • Determinator: Jordan refuses to let Perry walk away.
  • Easily Forgiven: Perry admits that of course he forgives Jordan, he just needed a minute to process what he learned.
  • Groin Attack: "Newbie, if the next two words out of your mouth aren't 'see ya' then the third will be 'oh my god my crotch you've punched me in the crotch."
    JD: See ya.
  • Improperly Paranoid: Although Jordan had ample reason to be concerned about their relationship. The fact is that not telling Perry was the father
  • Large Ham: It's Ryan Reynolds.
  • Misblamed: Spence points out that JD and Turk are blaming him for their failure.
  • Oh, Crap!: JD tries to run away when Spence spills the beans about Cox being the father, only to get caught in the tether of his saline drip.
    • Cox punches Kelso. Everyone is stunned. Except Ted, who just laughs until he cries.
    JD: That may come up tomorrow.
  • Papa Wolf: Cox is terrified he won't be a good dad, so JD points out that if Eliot were his kid, he'd definitely protect her from Kelso's non-stop abuse. Cue Cox straight up punching Kelso out, to everyone's shock.
  • Special Guest: Ryan Reynolds is JD's and Turk's college buddy, Spence.
  • This Is Reality: Because this show deconstructs medical dramas, we get another example of the banal reality of medical work with JD's opening monologue complaining about paperwork, the same three medications every patient gets, paperwork, mind-numbing monotony, and paperwork.
    • JD and Turk also get ripped to shreds for showing up to work having had a few beers.
  • Take That!: To some news items that were big at the time.
    Cox: NEWS FLASH. You can't drink and come to work! You're not airline pilots!
  • Team Dad: Cox has always kind of had this role, but he embraces it, if only for a moment, when he punches Kelso to protect Eliot.
  • Time-Passes Montage: We see the main three residents in the elevator. Their clothes change, their fellow passengers change, but their positions remain the same.
  • Ungrateful Bastard: JD's diabetic patient doesn't really care that JD saved his foot (diabetes can lead to peripheral neuropathy, which can cause extreme pain in the extremities).
  • When She Smiles: "You're a father, can you believe it?"
    Cox: Everyone have a good one. I'm going home to see my son.
