Much of this episode's first half is from the perspective of this week's patient, Lee (Yasiin Bey, better known as Mos Def).

Dr. House has an accident in Middletown and winds up in a different hospital, next to a patient that's already been dismissed as brain dead. But House suspects there's more to his case and moves the patient to Princeton-Plainsboro.

Dr. Chase prepares the patient for the brain biopsy. After the biopsy, the patient becomes unable to communicate by blinking, and the episode switches to a more normal perspective. Dr. Taub hooks the patient up to a brain-computer interface.

For hours, nothing happens. Then, just when Taub's about to give up, the patient moves the cursor up on the monitor. Once again able to communicate, the patient answers some yes or no questions.

We see Dr. Hadley administer some eye drops from the patient's perspective. She rules out cadmium poisoning, Dr. Foreman concurs.

Meanwhile, Dr. Cameron treats House for his injuries from the accident.

Dr. Kutner figures out the patient is trying to tell the doctors that his foot itches.

A discussion about Hadley wearing or not wearing a bracelet Foreman gave her leads Kutner to his "Eureka!" Moment. Taub takes credit for Kutner's idea, with which Kutner seems okay, but House sees right through the nonchalance.

Dr. Wilson figures out why House went to Middletown.
