- Straight: Alice's last name is never revealed.

- Nobody's last name is revealed.
- Alice's last name is never said, even when she gets married, and even though she's a doctor, her patients are inexplicably on a First-Name Basis with her.
- Downplayed:
- Alice's husband is Bob Smith and her father is Charlie Jones, making her last name either "Smith" or "Jones".
- Alice's children have the surname "Johnson", yet their father is absent for unexplained reasons. Therefore, it's unknown if Alice ever changed her maiden name to "Johnson" and, if so, if she changed it back (which she likely did if she was divorced) or not (which is more likely if she is a widow).
- We know Alice's last initial is R., but don't know what it stands for.
- Justified:
- Alice deliberately avoids mentioning her last name, whether due to bad blood with her father, having the same last name as a husband who died tragically, or being paranoid about others knowing her details.
- Alice 'Doe' suffers from amnesia.
- Inverted: Alice is always referred to as "Alice Maria Smith" every time.
- Subverted: In season two, Alice's last name is revealed to be "Wilson".
- Double Subverted:
- But then she gets married, and we're given no clue as to whether she takes her husband's name or not; she isn't even shown having The Maiden Name Debate.
- It turns out, "Wilson" was just an alias.
- Parodied:
- Zigzagged: Alice's last name is never revealed... until it turns out to be "Wilson" in Season Two... but that was an alias... her real last name is "Jones". Then, she marries Bob Smith, but it's left ambiguous as to whether she took his name.
- Averted: Alice is introduced as Alice Smith from the start.
- Enforced:
- Lampshaded: "Alice who?"
- Invoked: Bob, who has Medium Awareness, tells Alice not to let the viewers know her last name.
- Exploited: The villains can't find Alice since they don't know her last name.
