  • The very idea of Hi-Fi living dead are pure Paranoia Fuel. Like any other undead, they are obedient to their creator, but while the usual rank and file are just your regular shambling corpses, the Hi-Fi are practically indistinguishable from a regular person. And anyone could be one, and they could be anywhere.
  • The Necroanalysis of the Endless Summer owner. Basically a dissection of the still squirming living dead who seems to have no idea of what has happened to him and why this is going on, pleading for mercy all while So'un and Con Su just go on as if this is another day at the office (which it is) including cutting open his skull and chest cavity. While he is undead and would rot eventually, it is stomach churning regardless.
  • The way Tokyo decided to deal with its aging population was nothing short of repulsive with the creation of Elderly Disposal Zones. They are areas where gun laws are more relaxed and the elderly above 60 who can't pay their taxes are taken to be killed for entertainment. The whole slaughter is treated little differently from an amusement park complete with Repulsive Ringmaster's and everything.
  • The torture of the vice-governor at the hands of a necromancer. The story does not shy away one bit from the cruelty that was done to him with the torturer making sure to drag it out as long as possible before finally finishing him off.
  • In The Fortress, on top of being an already nasty Den of Iniquity, also contains specialized cells for Juichirou to conduct experiments on various humans his cohorts have captured, all in the name of improving his sexaroids by using the poor victims as samples and reference material. Some of the experiments done here are nasty to the extreme.
  • In the wake of the assault on The Fortress, Iria and Ethica are captured by Milgram with Iria then taken away and tortured by Pavlov with Ethica being left to listen to her screams. While the whole thing was staged, the effect it had on Ethica is nothing short of chilling with just how easily Milgram managed to break her.
  • And going on at the same time it is shown just the kinds of lengths So'un is willing to go through to save Iria with him brutally torturing Rorschach in some rather graphic ways.
  • In Kiriri's route we are treated to a wonderful scene of Ryouko getting lobotomized whilst being fully conscious, the necromancer responsible causally toying away with her brain and showing how easily a person can be broken by simply tampering a little at the right places.
  • The torture of Sophia in Kiriri's route is nothing short of nauseating where a living dead Kagekatsu is forced upon Hokoyasu's orders to torture her himself. But, unlike So'un, very little has been done to keep Kagekatsu in a fresh state meaning that he is decaying and rotting as he is forced to vomit putrid fluids down her mouth as well as forcing her to eat a stream of maggots. On top of white noise boosted the drug Synesthesia being used, by the end of it, the poor man is just as broken as his victim.
  • While Hokoyasu had always been established as a man who would go to some disturbing lengths chasing his beliefs, Kiriri's route makes it clear just how far he is willing to take it. This includes not only making use of Milgrams own undead, but turning the whole of the Military Police HQ into a necropolis, adjusting it to better serve the maintenance of Living Dead and even killing all of its staff to turn them into even more undead, all unfailingly loyal to him. Not even Kazuma was spared. And worse, he plans to unleash all of this upon Tokyo as a twisted for of Population Control.
  • A small moment in Mitsumi's route. When the Hawaii is sinking, Ikkatsu grabs hold of So'un and drags him down into the cold depths of the Tokyo Bay. The whole thing is in first person as So'un is struggling against Ikkatsu's grip as he is sinking into the dark abyss and struggling not to drown even with his enhancements. The effect is nothing short of unnerving.
  • The Necropolis Project. Milgrams ultimate goal, to turn what was supposed humanities hope for the future with the Tokyo Rebirth Project and turn it into a writing mass of decaying and rotten artificial flesh driven by pure despair and hate that will slowly consume everything around it. First Tokyo, then spread across the rest of the world, devouring everything it comes across until all life is extinguished. The soundtrack that plays when the thing arrives does not help.
