  • Code Lyoko: EG: While researching XANA's past schemes, Sunset discovers that this is one of its weaknesses, and explains as much to the rest of the Warriors in Chapter 25:
    Sunset: Think about it. XANA had a perfect plan keeping Fluttershy out of the towers when he possessed the bees. If he combined that with, let's say, that time in the gymnasium, he would have won easily, but he didn't. He never combines multiple attacks from past records, nor does he actually learn. He's just trying multiple things until he finds something that works.
  • Dinosaur: A Prehistoric Park Adventure has a heroic example in Extinction Lasts Forever. When Diablo breaks out, Irwin tries to attack him the same way he did Matilda. Unfortunately, he forgot that the staff had added a shock collar to prevent this kind of incident from happening again.

Batman

  • Cat Tales hangs a lampshade on this practice; all of the Rogues Gallery have struggled with 'Writer's Block' from time to time, having trouble coming up with new crimes that follow their signature themes.

Calvin and Hobbes

Danny Phantom

  • Averted in the Facing the Future Series where Technus tried to repeat his plan of taking control of the Cybertron satellite. As he stated, "A good idea is a good idea". Granted, this is one time where it would've succeeded this time as Danny couldn't use the same method he stopped him with the first time.

Harry Potter

  • In The Power of Seven, Voldemort notes that he's breaking this rule, with one critical adjustment: using real bait in Demelza. Ultimately, this change doesn't help; Harry and his bondmates not only confirm that the bait is real this time around, but construct a more detailed plan than the one they'd used the first time round.

Invader Zim

  • Averted in Season 2 of The New Adventures of Invader Zim, where Nyx proves her loyalty to Zim by presenting him with a datafile relating to one of his earlier plans, which makes him realize that he can modify the plan and make it work this time. What exactly that plan is, however, is currently unknown.

The Loud House

  • Reacting to The Loud House uses this to help explain Lynn's behavior in "No Such Luck". Lynn had gone on a winning streak by using the exact same tactics for every game. When her opponents caught on and adjusted accordingly, she started losing, leading her to start trying different "rituals" in order to win again.

Miraculous Ladybug

  • Feralnette AU: Lila learns why this rule is in place during the Birds of a Feather arc. During the events of "Ladybug", she successfully got Marinette expelled, then evaded any serious punishment even after the deception was revealed to Principal Damocles and Ms. Bustier. When Marinette opts to take the heat in Alya's place, Lila gloats about how she'll get her expelled for good this time. However, she's shocked to learn that Chloe of all people secretly took steps to ensure such a scheme wouldn't work; Principal Damocles calmly informs both girls that expulsion is not an option because Chloe says Marinette is tutoring her, and will bring her father's wrath down upon the school if he kicks her out again.

Mega Man

  • In Mega Man: Defender of the Human Race, this is played straight most of the time, but averted in episode 11 where Wily decides to repeat a semi-successful plan in the hopes of hurting Mega more.

My Little Pony: Friendship Is Magic

  • Averted in Winter Storm. Queen Chrysalis does her research before enacting another plan that hinges on the same scheme of kidnapping and impersonating a princess, learning from the mistakes she'd made the first time around. As a result, it works perfectly.
