• The protagonist admitting that he's very much worried about Sakura and wants her to live. She then hugs him, for real this time, grateful that he actually started caring for her.
• When the protagonist lists every single decision he has made that eventually led to him going to meet a classmate for fun on a weekend, something he would normally never even consider. To show how much he's changed by that time, he was originally going to write "I want to brew the dirt under your nails and drink it" in his last text message to Sakura, but then decided that "such trivial words cannot fully express our relationship", and changes it to the titular "I want to eat your pancreas".
  • For a bit of irony, in Sakura's will, she writes that she was going to finish with the exact same "trivial words", but decided to finish with "I want to eat your pancreas" for the exact same reason as the protagonist in his last message for her.
• The protagonist reaching out to Kyoko and asking her to become friends with him. The epilogue in the animated movie shows that she did.
• The epilogue of the animated movie has the protagonist visiting Sakura's grave. When he leaves, there's a mysterious wind that washes over him, making him look back and smile before departing. The ending reveals a single cherry blossom petal on her grave, implying that Sakura is looking over him.
