** I agree, but there are two things... one, the doctor said that one percent get ''no worse''... and Spider clearly does. However, two... in the Futuresight Reservation, the weird guy says something like "We think we licked that problem, anyway." So one of the two doses of Information Pollen may well have not been as hard on the system... increasing the chances substantially. (And hey, maybe the Futuresight Reservation helped him out after he started getting sick!)
** After that reveal, I took it to mean that Spider somehow faked getting worse (there's gotta be some way to fake nosebleeds using nanomachines, and the rest is just acting), in preparation for a forced retirement after toppling the Smiler. He hated working in the City after all, and what better way to do it than fake a debilitating illness. It's the perfect way to end the story, with Spider fucking with everyone, and finally getting them to leave him alone (mostly). It's possible that the Reservation didn't cure the illness but was able to curb the degeneration, and with Spider's ego, of course he'd think he was special enough to be in the one percent group.
** Given that the i-pollen was produced by futurist technologist friends of Spider, it's not unreasonable to think that they'd at least be working on a cure - there is a substantial time skip between the second last and last issues, after all.
