Why does the Outsider leader Marek have such shiny white teeth while virtually everyone else on the island, (Outsider or Insider) usually has bad teeth, even though he's been living in the same squalid conditions like everyone else has for the last seven years? The answer is simple. The Outsiders are the only ones actually receiving the regular supply drops, which would include very basic personal care things like toothpaste, floss, mouthwash and toothbrushes. As the leader, Marek would have first pick of the supplies and has clearly been making use of them.
