Decided: This is limited for split personality cases only. For cases of actual 2 or more beings in one body that are of different genders, see this one.

Split Personalities are weird things. They can have completely opposing personality traits to the main personality, different skills sometimes... But perhaps one of the oddest from a logical standpoint is when a split personality is actually a different gender from the main personality and from the body they control.

How exactly they rationalize this gender incongruity can vary. Some may simply ignore it. Some may claim to be transgender. Some may actually be shocked to find themselves in the alternate body. Every mental coping mechanism will be a brain-twister, however. Sometimes may overlap with Gender Bender, especially if The Mind Is a Plaything of the Body.

Compare Super Gender-Bender for when your other gender split personality has superpowers.

Anime and Manga
  • Shiki Ryougi from Kara no Kyoukai—at least, until a car accident kills off her male personality. Also, her entire bloodline is apparently afflicted by this disorder.
  • Kanon and Shannon from Umineko no Naku Koro ni (although Yasu's original gender is left ambiguous, and so which one's the OGSP is left to speculation). There were hints of this throughout the entire novel, but it's pretty much spelled out by the seventh Episode. This is the largest clue to solving the entire story; figuring this out blows most of the closed rooms open.
  • Hato, the Wholesome Crossdresser of Genshiken is male but has a female inside voice/spirit/personality/whatever, teasing him each time he is with Madarame, much to his annoyance.
  • The former spirit detective Shinobu Sensui from YuYu Hakusho has developed no less than 7 identities, one of which is a girl called Naru, who represents the last remnants of his innocence.
  • Kotonoha Nanase from Gun X Clover has seven different personalities which protect her primary personality, who has been targeted since childhood because of a unique ability to read dead languages. Her fifth personality is a Hot-Blooded male martial artist named Itsuba.
  • Invoked in Love Stage!!. The protagonist Izumi, an otaku from a prominent family of actors and music talents, is cast in a TV Drama as the main character, a rich young master with three personalities, one of which is a girl. This difficult role shows off Izumi's prodigious but heretofore unseen acting abilities, causing feelings of inadequacy in his senior co-star and boyfriend, Ryouma.

Film
  • A sub-plot in Session9 revolves around a psychiatric patient named Mary Hobbes, who suffered from dissociative identity disorder and had three "alters" named The Princess, Billy and Simon. Billy and Simon sound very masculine and have masculine names while The Princess sounds more feminine. The film leaves it deliberately ambiguous whether this is actually a case of dissociative identity disorder or Demonic Possession.
  • The main character from Doctor Jekyll and Sister Hyde is a man whose Hyde is a woman.
  • In the famous twist ending from Psycho it turns out that Norman Bates' mother is long since dead, but Norman has a split personality who he thinks of as his mother.
  • In Identity, the ending reveals that all of the ten main characters, consisting of seven males and three women, are alternate personalities of Malcolm Rivers.
  • An in-universe example in Adaptation., in which Donald's inane psychological thriller screenplay The Thr3e ends with the reveal that the cop protagonist, the killer he is chasing after and the female victim the cop falls in love with are all the same person. Donald chooses to cheerfully ignore all of the Fridge Logic and plot holes created by this plot twist.
  • Carter in Raising Cain has several split personalities, who are leftovers from the research he went through as a child. One of them is a quiet, intimidating woman named Margo, who takes over to make him escape the police, and go after the real villain of the story.

Literature
  • The Truth introduces Altogether Andrews, a beggar with no less than 8 different personalities, one of which - named Lady Hermione - is female. They activate at random, and can apparently communicate with one another. And none of them respond to "Andrews".

Live-Action TV
  • United States Of Tara is about a woman with DID named Tara Gregson and her multiple personalities, one of whom is a man named Buck who claims to have lost his "member" in 'Nam.
  • In a first season episode of Psych Shawn and Gus are hired by a client who turns out to have multiple personalities. One of them is a woman who has been trying to arrange a sex change behind the (male) primary's back, another is a murderous psychopath who kills the doctors the female alter has been going to.
  • In a House episode, a girl develops an alternate personality whom she considers her "boyfriend". Oddly enough, she tests positive for pregnancy and the doctors think he got her pregnant, which doesn't turn out to be true.
  • In an episode of The X-Files there was a woman who claimed to have a Split Personality; one of the personalities was male.
  • In an episode of Psych, Gus falls for an extremely attractive woman... at a mental hospital. Eventually, he learns that she is sharing a body with another personality named "Frank".
  • Buffy the Vampire Slayer: The alter ego of villainess Glory is the male Ben Wilkinson. However, if any human were to discover this, they would instantly forget.

Video Games
  • In Killer7, KAEDE is one of the many split personalities of Harman Smith and the only female one. The appearance and physical characteristics change to match that of the personality in charge though, and it's never made totally clear how these personalities came about in the first place as the game is one big Mind Screw.

question: is this for schizophrenic characters only? Or does it allow for instances where two people (of opposite gender) share the same body? For example:

    Anime

    • Tesutomu and the titular Birdy from Birdy the Mighty. Due to the unfortunate circumstances of their first meeting, Tesutomu ends up dead, since Birdy mistook him for the guy she was after and kinda fried him. So they end up having to share the same body while the Space Federation regenerates Tesutomu's old one. What's a guy to do when he's stuck inside a the body of a hotheaded Action Girl?

    • In Blaz Blue, Platinum The Trinity is female (presumably) parody of the Magical Girl archetype. She has three seperate personalities, two of which match her physical age; Luna (a bratty girl) and Sena (a polite boy) and a third Trinity Glasfille, one of the 6 heroes. It's implied that it's more a case of Sharing A Body though.

    • The entire plot of Heinlein's I Will Fear No Evil is that a man inhabits the body of a recently-deceased woman, only to find she isn't really all gone, leaving both of them in the body. Then at the end of the book, another dude inhabits the body with the already present consciousnesses.

Any example of Split Personality that I can think of includes at least one alter of the opposite sex. So then the idea of an opposite sex alter should just be added to the description of Split Personality. But really, this is chairs, as we'll have things like alters of different ages, as in a child inhabiting the body of an adult, or even a non-human alter like Tara's "gimmie".

Okay then. I think it's worth noting that Buck from United States Of Tara is a heterosexual male, even having sexual relationship with a bartender named Pammi. He also claims that his bits where blown off in 'Nam. Tara also later develops the personality of Bryce Crane in season 3.

I think it's worth distinguishing this from "person has an actual soul/spirit/mind of a different gender living inside them", but that might be worth launching as trope on its own (I can think of a few examples).

Maybe "multiple gendered" as opposed to "opposite gendered"? "Opposite" is a relative term, and it wouldn't necessarily be appropriate if, for example, a character has more than one alternate personality, one of whom is of the opposite gender to the original personality but not the others.

    • Mari from Inside Mari is implied to have three personalities: The female "Mari", the female "Fumiko", and the male "Isao". Fumiko is her natural personality however Mari developed due to childhood traumas and her mother forcibly changing her name. Isao is the newest personality and developed due to her interest in the real Isao. The Isao personality is the main character of the manga and believes he is the 'real' Isao stuck inside Mari's body.

Also Umineko goes under Visual Novels.
