Phil has some ideas how to "optimise" hospital. Kisegach at first gets enraged by his boldness, but then realises that she can use him against Bykov (who previously offended her), and sends him to order Bykov around. Bykov quickly gets tired of Phil and tell him that this is his sexual game with Kisegach: she is aroused by Bykov being angry at her. Phil falls for this and stops working for Anastasia when he starts seeing "proofs" of that statement.

Orderly Pasha asks Lyuba for help: he wants to hit on Varya, but doesn't know how. Unfortunately, Varya sees them together and now thinks that they are in relationship. Varya thinks that Lyuba is ashamed of Pasha, and decides to "help" them. This culminates in them attending a restaurant together... only for them to learn that neither Lyuba nor Varya do, in fact, like Pasha. He takes it surprisingly well.

Lobanov makes a mistake of insulting his patient (a female professional boxer), because he considers women to be unfit for sport. When he angers her enough, she accidentally hits him in the eye, leaving a visible bruise. Gleb doesn't believe him that said woman is indeed a boxer, and tries to blackmail him into staying away from home today (so Gleb can invite his new girl), by threatening to tell this to Lyuba, who would spread this story across the hospital. Semyon agrees, on condition that Gleb would take that boxer as his patient. And then Gleb makes all the same mistakes as Lobanov before him, and gets hit in the eye, too.

!! This episode provides examples of:
* ADayInTheLimelight: For orderly Pasha, who wants to hit on Varya. Unfortunately for him, Varya mistakes his conversation with Lyuba for them being in relationship.
* EskimosArentReal: Neither Semyon nor Gleb believes that female boxers exists for real. They finds out the hard way that they actually do.
* HoistByHisOwnPetard: Bykov mocks Kisegach when she tells him about Phil's plan to "optimise" the hospital and (jokingly) tells her that Phil may be right and she may indeed be bad as the hospital administrator. She gets angry, and decides to give Phil the full authority, and specifies that he must be particularly persistent with Bykov.
* InLoveWithYourCarnage: Trope. When Phil comes too far in his attempts to "optimise" hospital and annoys Bykov too much, Bykov tells him that the whole situation is his "sexual game" with Kisegach: she's aroused by him being angry. Phil falls for it and when he tells her about their conversation, he starts noticing that Bykov may be right (though it is not clear wether it's true, or Phil's mind is screwing with him), and stops working for Kisegach out of squick.
* IWasBeatenByAGirl: Lobanov is ashamed by his patient (female boxer) kicking his ass after he insulted her, and Gleb tries to use it to blackmail him into staying away from home today (Gleb has... plans for that night). Semyon agrees, on condition that Gleb would take this patient... which leads to her kicking Gleb, too, for same reason. Now they both asks Bykov to not tell anyone... which he immediately ignores and tells everything to Lyuba.
* LaserGuidedKarma: Gleb mocks Lobanov for IWasBeatenByAGirl being beaten by a woman (he doesn't believe that she was a boxer, cause, come on, female boxers?), and tries to blackmail him by telling this to Lyuba, the main resident gossiper. Semyon asks him to deal with that patient himself, which leads to Gleb himself being beaten (due to acting the same way as Lobanov), and Bykov actually telling to Lyuba.
* MuggingTheMonster:
** Lobanov makes a mistake of insulting his patient (female professional boxer), because, apparently, women are unfit for "serious" sport. This leads to her accidentally injuring him (she later apologises for her impulsiveness). Gleb later makes the same mistake, with same results.
** When Phil turns Bykov's gaming console off in the middle of play and tells him that Bykov is forbidden to waste his working time on video games, Bykov shouts on him and tries to force him to eat the document which apparently allows him to do so... but then Phil reveals another copy and tells him that this document is, in fact, real, and signed by Kisegach, so Phil can (and would) fine him for every violation. However, when Phil comes too far, Bykov does find a way to retaliate.
* PoorCommunicationKills: Orderly Pasha wants to hit on Varya, and asks Lyuba for help. Unfortunately, Varya had seen them together and now thinks that they are in relationship... and that Lyuba is ashamed by Pasha, so she decides to "help" them. This leads to awkward situation when they (all three) comes to restaurant, and Varya accidentally asks "why they come here, if no one likes Pasha?", after which Pasha decides to just leave.
* StayInTheKitchen: Semyon tells his patient (professional boxer) that women are supposed to cook for their husbands, not attend "men's sport". When she understandably objects, he starts arguing with her (without providing any real argument for his position). When he angers her enough, she just hits him in the eye, strongly enough to leave a visible bruise (she apologises to him, because she'd done that impulsively... right in front of Gleb). Gleb (who doesn't believe Semyon and takes this patient from him) later makes the same mistake, with the same results.
* TyrantTakesTheHelm: When Phil receives authority to "optimise" the hospital's work and fine everyone who refuses to abide new rules, he quickly enrages everyone, by taking away everything they likes (video games and ABoyAndHisX Igor from Bykov, cognac from Kupitman, gossips from Lyuba...).
