We never actually see Theodora's skin ''turning'' green, her nails becoming long and sharp, etc. Rather, she falls to the floor behind a table and a green, taloned hand reaches up to claw its surface. It's just as plausible that Theodora's original body slipped into a magical coma, and a corporeal embodiment of her new DrunkOnTheDarkSide aspect - the Wicked Witch of the West - manifested out of thin air ''above'' her unconscious form. The two evil witches hid her old body somewhere safe and secret, where it remained until Dorothy melted the "Wicked Witch" manifestation, breaking the spell and waking up Theodora's old self in the process.

** Actually, the apple withering her heart is actually somewhat inline with what the original book series said about the wicked witch of the west, that she was so evil, all her blood had dried up ages ago. This movie, legally speaking, is supposed to be a prequel to said book. But still a pretty interesting WMG.
