[[WMG: Johan has hypnotic powers in his voice and possibly his eyes.]]
He's able to convince ''countless'' people of varying moralities to do ''horrible'' things - both to themselves and to others - all with seemingly nothing but his charisma and his good looks. And yet, ''how'' is he able to do so? Sure, he's an expert manipulator but his track record is a little ''too'' perfect in that regard. But if you believe that he has hypnotic abilities as well as his good looks, it suddenly makes perfect sense! Not to mention that this could possibly explain the final scene between Tenma and Johan in the series. If Johan hypnotized Tenma, most likely when he first sat up after supposedly being asleep and Tenma made eye contact with him, he could've used his hypnosis to give Tenma that flashback or "hallucination" about Johan and Anna's mother. And the fact that people like Grimmer, who himself was a victim of abuse and experimentation at Kinderheim 511, got superhuman-like powers in the form of "The Magnificent Steiner", only serves to support this theory!

He was playing the game that Martin (the CreepyChild, not the noir-ish ganger who befriends Eva) was trying to get Dieter to play: keep putting yourself in increasingly dangerous situations to see if you win (surivive) or lose (die). As Johan sees it, the more he wins the game, the more he becomes something...[[TheUbermensch far grander than an ordinary human]], something that would know no fear or remorse (and therefore know no emotional trauma either). His horrible past has utterly broken him (we've seen how he reacted to looking at that picture book) and becoming something beyond human is his way of crafting a new identity and utterly forget his past. He never wanted to die, but he kept increasing the odds that he would so that he'd keep "ascending" to this new nature/identity with each game.

He didn't avoid that truck on the way to the hospital. He collided with it head-on, causing severe head trauma that put him in a coma. Furthermore, the fact that he gets hit by a car twice over the course of the series is not a coincidence. That's his subconscious reminding him of the accident.

This explains why Tenma becomes almost too perfect as the series progresses. What we see is not who Tenma actually is, but an [[MarySue exaggerated, idealized version of him]]. In his coma-induced dream world, he gets to live out a fantasy where he can save just about everyone with his medical skills and live up to his moral standards. It's all an attempt to process his guilt over his decision to blindly follow orders and abandon his patient, the Turkish construction worker.

Other issues are also addressed in the dream. Several characters represent aspects of Tenma's psyche, and his interactions with them reflect his internal conflicts. Johan is his [[ShadowArchetype shadow]], representing everything he dislikes about himself, particularly his repressed anger and his capacity to harm and manipulate others to get what he wants. Nina, being [[NiceGirl the good counterpart]] to [[EvilTwin Johan]], represents everything good in Tenma as well, and his desire to protect her innocence reflects his own desire to maintain his integrity. Dieter is his wounded inner child, the part of him who feels [[ParentalIssues abandoned]] and [[TheUnfavorite rejected]] by his parents. Grimmer is what he could become [[BewareTheNiceOnes if he keeps denying his anger]] and continues to be a [[ExtremeDoormat doormat]].

In the end, when Tenma tells Johan his real name, that's him affirming his shadow's existence and telling himself that his parents loved him, and is still worthy of love. However, when Johan asks which child was unwanted, it still shows doubt on Tenma's part. He doesn't know if the “bad child” (Johan) was truly loved, or if the “good child” (Nina) simply suffered for no reason.

Still, Tenma seems to accept this reality and finds the strength to move on. When we see the empty bed, it means that Tenma has awakened from his coma, ready to face his life.

Essentially Yang Wen-li is a distant descendant of Tenma, while Reinhard von Lohengramm is either descended from Nina or Johan and Paul von Oberstein is a descendant of Lunge.
