At first glance, the game itself is mostly the same as the original, but with a major revamps. The first major thing is that the translation is more direct of the Japanese version (eg, Pokey is Porky, Giygas is Gyiyg, the Runaway Five are the Runaway Brothers), but still maintain some elements from the English version (eg the Pencil and Eraser Eraser and the Runaway Brother maintain their Western outfit colors). But one of the main change is that the difficulty has been tweaked to be slightly harder compare to the original, making it a NewGamePlus version for those who played the original version. Though it fixed a few issues with the original to make it a bit easier (eg Jeff's inventory space being more organized). So it's mostly trading off some stuff compare to the original game.
