* An ElephantInTheRoom is addressed between the two Doctors and Donna in the TARDIS: The Doctor has been running themself ragged ''for centuries'', maybe even ''millennia'', and is so traumatized by what they've seen and what they've done that they're running on fumes. Despite the Fourteenth Doctor [[BlatantLies trying to assure the two he's fine]] due to being ''"post bi-generation"'', the Fifteenth Doctor proceeds to list several events in his life that have emotionally impacted him the most, with [[Characters/DoctorWhoSecondDoctor Two's]] [[Recap/DoctorWhoS6E7TheWarGames trial and exile]], [[Characters/DoctorWhoFourthDoctor Four's]] [[Recap/DoctorWhoS16E1TheRibosOperation Key to Time adventures]], [[Recap/DoctorWhoS18E7Logopolis Logopolis' destruction]], [[Recap/DoctorWho50thASTheDayOfTheDoctor The Last Great Time War]], [[Recap/DoctorWhoS31E13TheBigBang The Pandorica]], [[Recap/DoctorWhoS3E4TheDaleksMasterPlan Mavic Chen]] and [[Recap/DoctorWhoS25E4TheGreatestShowInTheGalaxy the Gods of Ragnarok]] being notable examples. The whole list can elicit a feeling of horror in the audience, because each viewer realizes that every time an incarnation of the Doctor happily left a planet and travelled off with the TARDIS to new adventures, they were unknowingly bottling up all the trauma they had faced and subjecting themselves to new ounces of painful experiences. ''And it's reaching critical mass here.''

** The Doctor's reaction when he discovers that the Time Lords are returning when the Master tells him. He just looks so heartbroken and exhausted. And keep in mind, he had to deal with Daleks, The Master and Davros returning in prior series. Not even them returning had the Doctor this worried compared to the return of the Time Lords.

** You can just ''feel'' his rage and anger about having to die, especially after having cheated it several times (most notably in "The Stolen Earth"). Compared to the end of Nine and most other Doctors before him, he very much feared death because there was still so much left undone and he was full of guilt and regret, but ultimately had to accept it in the end. It's comparable to what many real-life people go through when they are dying, and it only makes things so much sadder.

*** A ''very'' interesting point when you realize it doesn't even end there. Eleven is forced to deal with the impending explosion of the TARDIS, his very complicated rescue from the Pandorica, the precise moment of his death, and now The Question itself all in the form of vague prophecy. You even get one more rage against the universe when Eleven declares he can still do anything he wants because he has all the time in the world, and ''"time has '''never laid a glove on me!'''"'' A fury quickly punished when he discovers that the Brig has just passed away. Damn you, Time. Damn you.

If you watch this episode for the first time, you truly realize that despite Nine and Ten commenting about the wonders of Gallifrey and the Time Lords, they truly had become the greatest threat the Doctor had ever faced up to that point.

*** However, this was actually touched up upon on a mini episode called "P.S." where Brian is given a letter sent to him by a man called Anthony (later revealed to be his grandson). The letter explains what happened to them and Rory apologies that he will never be able to see them again. Despite obviously not being able to tell him directly, its nice that Rory thought of Brian and knew he would need an explanation.

Added DiffLines:

* Ruby's reaction to being told that the program trying to genetically track down her biological parents failed. She is clearly holding back tears and putting on a brave face as she shakily tries to ask whether they could keep looking or there's another way to trace them, as well as bringing up how unusual it is that there was nothing traceable at all. And this happens on her birthday too.
** Later on Ruby brings up the news to Carla to avoid having to explain the Doctor's presence and the entire ordeal they just went through to save Lululbelle. But it's easy to also see that Ruby's breakdown might not have been entirely faked given she didn't get a chance to process the bad news before having to save Lulubelle from the goblins, not to mention she'd clearly hit her limit for the day already when she had her first outburst.

Added DiffLines:

* The Doctor tells Ruby and Carla that he was abandoned and adopted and that he has no family. After the previous special where the Fifteenth Doctor told his predecessor that they'll get to heal and "fix" themselves from their trauma, it's clear that even with all his cheeriness and vivacity Fifteen still has a lot of unresolved baggage regarding his origins as the Timeless Child.
* Also crosses into Fridge Sadness, as Fifteen saying that he has no one and no family seems contrary to the happy ending Fourteen got by settling down and living out the rest of his life with the Nobles. Unless from Fifteen's point of view, he managed to do just that - he lived, and the Noble family are now all gone in his time.
