Bykov considers celebrating the birthday of his old friend, Boar, to be so important tradition that he ''can't'' stay with his son Ilya when asked by Kisegach, especially since she didn't warn him beforehand. Knowing that he wouldn't do as she wants voluntarily, she steals his passport, leaving him no option but to do as she wants. When Bykov comes to the hospital, he announces that Sophia would be accepted by default, while the rest must compete for remaining two places by earning dog medals for pleasing Bykov; whoever wins the most, would pass themselves and would be able to choose a second partner from the losers. Alternatively, someone may try to steal Bykov's passport from Kisegach and win a place that way, but only Maxim takes the risk. As for Sophia, she's now an outcast, because everyone thinks that she exploited her relation to Kupitman in order to cheat her way into internship over their heads.

Alexei is first to realise that only things that matters is what Bykov would order them, and receives the first medal. Vakhtang is first to notice the smell of acetone -- and to guess that it may mean diabetes -- and earns a third medal. The fourth medal gets (re)earned by Zhanna, who, when Bykov gives them a trick task (collect fecal matter), ends up being the only one to ask ''whose'' matter, before going anywhere. Meanwhile, Maxim tries to steal Bykov's passport from Kisegach's office by impersonating electrician... only to fail to find it. But it seems that she has a safe, which he never tried. He later tries again, this time asking Kisegach to open the safe, but she recognises him as one of interns, forcing him to run. While he fails at his task, Bykov awards him a medal solely for his courage. By the end of the day, both Zhanna and Vakhtang earns three medals each, while Alexei has only two. Maxim gifts him his only medal, on condition that if he wins, he would pick Max as the partner. As a tiebreaker, Alexei returns Bykov his passport, which he obtained by... annoying Kisegach by talking about his hometown till she snaps.

Phil then insists that Gleb changes girls more often than his socks, and that he would break Sophia's heart, but Kupitman, too stressed by this point, just yells on him that he '''doesn't care'''. Comes the night, and Phil hears some clearly erotic moans from the room, which means that Gleb ''already'' seduced Sophia. He calls Kupitman, who, despite (or maybe ''because'') of being drunk, rushes to them, only to learn that it's some completely random girl. As Gleb reveals, Sophia was too wasted, and he called a taxi for her. The sole one who's happy in this situation is Phil, who now knows that deep inside, his boss ''does'' care about his niece.

There's still only three places in internship... and five candidates for them. And since he wouldn't kick out Sophia no matter what, they actually have ''two'' places. In order to decide who would stay and who isn't, Bykov decided what he would use (dog) medals to reward their successes. Whoever wins the most, would receive the place in internship... and would be able to choose second teammate by himself. Alternatively, anyone may try and return Bykov his passport. Maxim was only one to actually try this option.

Sophia is now outcast, because everyone thinks she exploited her relation to Kupitman in order to cheat her way into internship over their heads. But it seems she may find a new friend... in Gleb, who had similar problems due to being a son of Kisegach. Sophia immediately jumped at opportunity to learn more about Big City... mainly, where she may find some interesting clubs.

Bykov proceed with his tests. First test was on "intuition": were interns would go first -- to patients who needs it first, or most recent patients? Alexei was first to realise what only things which matters is what Bykov would order them, and received first medal. Second medal was earned by Zhanna, who correctly described required first procedure before visiting patient -- cleaning hands (or paws)... only to instantly lose it due to angering Bykov with stupid question. Vakhtang was first to notice smell of acetone -- and to guess what it may mean diabetes -- and earned third medal. Fourth medal was (re)earned by Zhanna, who, when Bykov gave them trick task, was first and only to realise what while Bykov ordered to collect fecal samples for testing, he didn't specified from ''whom''.

Meanwhile, Maxim tried to steal Bykov's passport from Kisegach's office by impersonating electrician... only to fail to find it. But it seems she has a safe, which he never tried.

Sophia, who plans to go in night club with Gleb, told Kupitan what she would be late, if she would go home at all. He doesn't care -- but Phil is, and he reminded Kupitman about promise he gave to Sophia's parents. But, Kupitman only bothered to ask bare minimum, and when learned about Gleb, decided what nothing bad would happen.

Maxim tried again, this time asking Kisegach to open safe, but was recognised and busted. While plan failed, Bykov was impressed by his courage, and awarded him free medal.

Kupitman thinks what he has nothing to hide and can just finally drink... but then Phil reminded him what every time Gleb goes in night club with a girl, it ends with them having sex. When Gleb assured him what ''if'' this happens, it would be mutual, and Kupitman has nothing to fear. After this Kupitman told Phil to not add anything more, because he doesn't want to know. Phil proceed to point out what Gleb changes girls more often than his socks, and he would just broke her heart, but Kupitman, by this point too stressed out to think straight, yelled at him what he '''doesn't care'''.

Everyone has three medals, except for Maxim (okay, Alexei has two, but Maxim gifted him one his, and since there's no such rule, Bykov let it slide). But Alexei ''also'' obtained Bykov's passport (somehow), and may choose second winner. It would be... Maxim, who made it possible. However, we later learn what ''both'' Kisegach ''and'' Maxim agreed due to Alexei ''not shutting up'' about his home town, Torzhok, and them simply wanting to get rid of him.

Phil clearly heard some erotic moans from one of rooms, which means, Gleb ''already'' seduced Sophia. When he warned about it Kupitman, he, despite (or, maybe, ''because'') of being drunk, rushed to them, only to learn what it was completely random girl. Sophia was too wasted, so he called taxi for her. Only one who's happy is Phil: he considers this to be a sign what Kupitman ''does'' cares about his niece.
