In flashbacks, Kate marries a friendly police officer named Kevin, who doesn't know she is a fugitive. She calls Edward Mars, who offers to stop chasing her if she's really capable of settling down. However, after a pregnancy scare, Kate realizes she can't stay with Kevin, and leaves him.

On the Island, Locke tells Sayid that Eko was killed by the Monster. During his burial, Locke sees a line of scipture on Eko's stick, which he interprets as a message to head north.

On Hydra Island, Jack refuses to operate on Ben's tumor. Alex attacks the Others while Kate and Sawyer work, looking for Karl. Juliet tells Kate that Pickett wants to kill Sawyer, and convinces her to persuade Jack to do the surgery. Jack still refuses, but changes his mind when he sees surveillance footage of Sawyer and Kate having sex in the cages. During the surgery, Jack deliberately makes an incision in a vital organ, and tells the Others he will let Ben die unless they allow Kate and Sawyer to escape. Tom interrupts Pickett as he is about to execute Sawyer, and makes him give Kate his walkie-talkie. Jack yells at her to run.
