** What's even worse than ''that'' is that [[spoiler:Stanley is actually dead. A phone call Heather receives wishes her a happy birthday and tells her that Stanley's new name is "7". Go to the basement where you have to solve the stretcher number puzzle and stand by the one with a 7 on it. That faint voice you just heard? That's Stanley whispering Heather's name.]]

In a series with some of the scariest ''things'' in gaming history, many people consider Silent Hill 3 to be the most terrifying out of all of them. ''With '''very''' good reason.''

** The room where you pick up the SMG at the end is frightening, too. The blood trail leads down to it, and in it you find the SMG in front of an inoperable elevator. The walls and elevator doors are smeared with blood and bullet holes, and the SMG itself is empty. The way the bullet holes are arranged, it looks like someone (potentially the someone in the wheelchair) started firing at something in the elevator, before the weapon got knocked out of their hands. Who was this person? Where are they? What were they shooting at? True to tradition in this game, [[NothingIsScarier you never]] [[ParanoiaFuel find out]].

* The Otherworld mall with it's dark corners and unnerving music is pretty scary.

* Some of the ways in which the monster attack and kill Heather are truly brutal; there is a running theme of maternity and femininity in Silent Hill 3, as the conflict of the game involves the cult's attempts to force Heather to give birth to their God. Hence, most of the monsters will attack Heather in sexual ways, or otherwise focus their attacks on her genitals and sexual organs.
** The dog-like Double Head, for instance, will grab Heather in between her legs with its teeth and begin to savage her groin and crotch. This attack does massive damage to Heather.
** The Closers will unfold their arm spikes and attempt to impale her; if she is low on health, a Closer will grab her, lift her up and position its spike in between Heather's legs in preparation to kill her. It will then impale her in the crotch and twist its blade, slicing apart Heather's genitals and reproductive organs. Heather will scream and writhe as blood pours out of the ruined region, before eventually falling still and dying impaled on the Closer's spike.
** The Slurpers are fond of ramming into Heather and knocking her over onto her back. They will then slide up her short denim skirt, and thrust their tapered heads directly into Heather's crotch. The knockdown attack does only minimal damage, but the follow-up attack on Heather's underparts does extreme damage and will kill her easily.
** The Scrapers are armed with what look like oversized hooks and spikes, which disturbingly resemble abortion tools. In further support of that resemblance, they will sometimes attack Heather by stabbing her in the groin, or swiping their hooks upward in between her legs, doing very high damage to her. If Heather is killed by one of these attacks, she will shriek and fall to the ground, curling up in pain with her hands in between her legs, holding her crotch. After spending several seconds whimpering and twitching on the ground, she will fall still and die.

* A decidedly different brand of [=HONF=] from [=SH3=]: while in Brookhaven Hospital, you find all these notes written by a guy named [[StalkerWithACrush Stanley Coleman]], and there's always a little doll next to them. The messages mainly consist of him saying he loves Heather, how he can't wait to meet her, asking her why she won't take the doll he left for her, and all sorts of other creepy, stalker[[BuffySpeak -y]] stuff (with the occasional hint for a nearby puzzle).

* The carousel. It starts off creepy enough, with those weird, poison-breathing flesh horses that you have to kill to progress. But when you do, the carousel starts sinking into the ground, and the horses turn into what I can only describe as ghost-horses, while that creepy Otherworld music plays. And then the [[MirrorBoss Memory of Alessa]] shows up...

* Some of the ambient noises can be pretty freakish. I became absolutely terrified of empty, silent rooms, just because LOUD noises like demonic yelling and chains rattling could suddenly assault my ears and make me almost fall out of my chair.

Pipe-wielding Nurses will reach up Heather's skirt and pull her white panties off, then spread her legs open wide, baring her genitalia. They will then forcefully ram their pipe into Heather's vagina, causing a spurt of blood to spray out of the dead girl's orifice. Pistol-wielding nurses will also remove Heather's panties and spread apart her legs, before shoving the barrel of their pistol into Heather's vagina and firing, causing Heather's body to jerk, with blood and gristle spraying out of Heather's crotch. On the third shot, the bullet bursts out of Heather's neck in a spray of blood.

Knowing that it's [[spoiler:Valtiel about to drag her dead body off to complete the rebirth of God]] lessens the impact, but to someone who hasn't played the game before, the brief, threatening possibility of rape to the main character in the opening demo will make you terrified of finding that monster.
