In the end, we (and Ryuuko) find out why Satsuki kept the extent and diagnosis of her illness a secret, the reason being because [[spoiler: she was dying of heart failure and the doctors offered to place her on a list to receive a heart transplant, even if there were no donors available, to which she declined, knowing she'd die before a donor is found and not wanting Ryuuko to kill herself, so the latter's heart could be used in a transplant]].
