* Perhaps Aperture did initially close off the shaft in 1961, but ended up returning to it anyway (making use of wasted space + and the financial difficulties requiring them to make better use of the space they already had). It could be the 50s section specifically that was considered out of bounds, or perhaps operating in a legally defunct area made it easier to hide their activities from the law during this period.

** As stated above, it's more likely the processing and production of raw moon rocks into Conversion Gel that releases toxic dust. Like how solid asbestos is technically safe until it gets broken up, the liqueous gel is probably safe too.

** Perhaps the neurotoxin generator has its own robotic maintenance team that were sent in to put the thing back together in the timespan in-between its destruction and Wheatley's fight. I take it that the facility still has other robots still doing their jobs fine, they just can't all collectively compensate for Wheatley's piss-poor management and lack of top-level administrative action (say, only the mainframe may be permitted to operate the reactor).

*** Maybe after Cave Johnson succumbed to his poisoning, a change in upper management meant a sharp reduction of the short-sighted decisions that brought them down to begin with, allowing Aperture to start operating slightly (keyword slightly, it's still Aperture after all) more sensibly, resulting in the company becoming relatively stable.

** Well, the pipers in the modern facility aren't actually pipes meant for the gel. They are transport tubes that transports various things throughout the facility (which includes you and Wheatley at one point, and deadly neurotoxin, as GlaDOS tries to use it when she has you trapped). It could be possible the tubes may have previously been attached to the pipes in the older facility, much like how the newer facility was still attached to previous versions of it. Due to Chell activating the pumps for the gels, those tubes began to pump the gels upwards into the newer facility. So, the pipes were always attached to the new facility, it's just that no one ever thought that anyone would ever go down into the older sections of the facility to turn the pumps on. And eventually, the workers of the newer facility were unaware those pipes were connected, as well as the existence of previous facilities.
