Despite having planned to fake his death, All For One was still terrified when All Might killed him the first time, only being brouhgt back to full health by the Doctor afterwards, and during his grand speech to the other High Generals, he makes it clear that when the Day of Genesis comes, he will be the one to deal with All Might.

While in the manga and anime they're for medical purposes, here on the other hand it is clearly for intimidation factor, and allowing him to fit in with the other more theatrical looking villains.
