Ratko came to Trevor seeking a fight, and Trevor didn't even come close to hurting him until he was fatally distracted. In a fair fight, he would've destroyed Trevor with minimum effort given both his pragmatic tactics and actual skill.

Ratko isn't a big bad wannabe, he has no ambitions of global conquest. He's perfectly fine letting Varney and Dracula scheme or "give handjobs" for eternity while he goes out there and kills at his leisure.
