During the filming of the 4th Harry Potter movie, when Daniel Radcliffe filmed the under water scenes, "thumbs up" meant "I'm drowning, get me to the surface". He forgot about it, and wanted to signal that he was okay, which the crew mistook as a drowning alert so they quickly hustled him to safety.
Think that qualifies as a Real Life example? If so, feel free to add.
