A variation appears in Kodomo No Omocha, where the relationship is between two teachers who aren't allowed to be dating. Their student Akito Hayama takes a photo of them making out and blackmails them both with it; actually, the main conflict starts when Sana Kurata calls him out for this (among other stuff), being the first ever to do so.
DearS features Mitsuka Yoshimine, a shameless exhibitionist English teacher who likes to show off by coming into class wearing extremely suggestive and revealing outfits, administering "punishments" such as her taking off an article of clothing (she said while wearing less fabric than a cotton swab), and publicly talking about orgies to the students. She even went so far as to prance about completely naked in a public bath in front of TV cameras. Despite all this, the students are more annoyed than aroused by her antics.

Literature

What guides the part of Greg Iles' Turning Angel is the controversial relationship between a physician and a not-quite-eighteen senior at a local prep school. When her body washes up at the beginning of the novel, the doctor asks the narrator for help in proving his innocence. Of course, the girl in question is Wise Beyond Her Years.

Theater

The plot of How I Learned to Drive, if a driving teacher (who is also the underage-at-the-time main character's uncle) counts. Nothing like theater for squick!
