There's a claim that people from after the war ignore how blood transfusions work. This kind of ignorance seems to be true for crop rotation (you can teach that to the former inhabitants of Vault 11 in Fallout 1), but doesn't really make sense in this context. Ignorance about this makes more sense from people who lived most of their lives in vaults, where they don't have fields. Also, the Vault Dweller didn't come up with it themselves, but rather knew about it because they were smart and had access to technical readings in the vault. People coming from vaults have both rich technical knowledge and cultures. Specifically, they have rather advanced medicine. Blood packs are usually kept in first-aid kits in vaults, and surgery and birth are common, and both tend to imply transfusions. Further, there are people who lived before the war all over the place: most ghouls experienced the war first-hand. I tend to think of the gulping sounds as an oversight, except in the case of the perk, where it does make sense to assume the Lone Wanderer is drinking the blood. It is strange tho that it heals so little. I'd understand the whole "I'll drink this!" thing for INT < 4.

Being so old I don't expect this to be updated but I just wanted to mention something for clarity's sake. There seem to be a few misconceptions about the Legion as a group. 1. The only people we hear say that they tolerate homosexuality are people who are staunchly Anti-Legion and are probably relying on rumor. 2. The Legion doesn't enslave and rape women in their peaceful territories (I.E, Arizona), the way the Legion operates in New Vegas is their wartime economy, men are killed or recruited and women are put to work as slaves. I remember some developer commentary mentioning that if the game hadn't be rushed as hard as it was, we would have gotten to see or hear about how the Legion lived in peacetime. They (I wanna say J.E Sawyer but I'm not sure) mentioned that women in Legion communities had jobs as nurses and teachers while men handled physical labor. Of course, none of this is communicated very clearly thanks to the game getting only half of the needed development time.
