The difference between Ax-Crazy and Insane Equals Violent

So, the Ax-Crazy trope is for characters that are deranged and prone to acts of random violence, and the Insane Equals Violent is when any mentally ill character is potrayed as violent. Considering how the latter trope is both controversial and inaccurate, while the former doesn't seem particuarly controversial (there are a lot of acclaimed villains that count as Ax-Crazy), how can I tell in writing when it's a case of the former or one of the latter? How does one write an Ax-Crazy character without falling into Insane Equals Violent? Any advice and tip is welcome.

Insane Equals Violent is likely what should be listed as a Trope in Aggregate, in that it's not really a direct analysis of a character itself, but rather a particular trend on how a work/series/genre/medium depicts characters in a general phenotype. A character who is unpredictably crazy and dangerously violent would be Ax-Crazy but not necessarily themselves demonstrate Insane Equals Violent — however, if an entire work/series/genre/medium depicts a vast majority of "crazy" characters as dangerously violent, that's what Insane Equals Violent is meant to illustrate.

Insane Equals Violent doesn't appear to be listed under Tropes in Aggregate, but it probably should be imo.
