[Discussion] Supplanting Or Co-Opting Real World Locations

Hi everyone. Opening a new thread to allow for a longer discussion than the random questions thread, as there isn't a "right" answer per say.

My question is pretty much as in the title. When making a story set in the "real world", sometimes a real location may possess some of the traits that your story needs (e.g. the county seat/town where the story is set in) while lacking other traits that are needed (e.g. it lacks an airport, has a small population, etc). In instances like these, is it better to supplant the location in its entirety and replace it with a fictional one or co-opt said real location and alter it to suit our needs.

On a somewhat related subject, in terms of real locations, what do people do about recognisable landmarks and organisations - e.g. district hospitals, police departments, etc? Do people replace these with fictional ones or use the real ones?
