Honestly, the Church was so corrupt by that point that he could get away with it. He wasn't even officially married to Vannoza despite her higher status as the mother of his children.
I suppose the younger lady could be taken to be his daughter or something, but let's not forget that the rest of the page serves as context too. We're not looking at it in a vaccuum.

How is it distinguished from Your Cheating Heart? The Mistress doesn't simply refer to an extra-marital paramour, but one who enjoys a specfic social status.

What has to come out is that the extra-marital relationship is an ongoing one and not merely some one time roll in the hay. The intimacy of the two characters on the right is supporting that aspect, IMO.
