I don't think it harms the page in any way, and it can help a little bit to understand the trope. That horse is a little bit out of place, which suggests it's not there naturally. It's not much, but it's something.
I'm also naturally skeptic to anyone who claims people won't understand something if they're not familiar with it while the claimer is in the know themselves, since I've heard that so many times about things I don't know about, but they make perfect sense to me.

Moon◊Nothing indicates that it was purchased, nor is there any indication that it was downloaded digitally. It's Tell, Don't Show at best. Also, it does not feel out of place, as it's the only thing in the image. Now, if there were several, actual horses next to it, then it would feel out of place.

If the image didn't have the speech balloon, or if the speech balloon was the caption rather than in the image proper, it would have been brought to Image Pickin' much sooner; it's the dictionary definition of Just a Face and a Caption.

Mechanical animals generally do not look like animals literally made out of guns, so that's kind of a weak defense.
And JAFAAC? Well, sure, if the dialog wasn't there. Except, the dialog is there, so the image illustrates that it's DLC on its own - even if it's telling instead of showing. Still better than nothing and not actively harming the page.
Moon◊It says right on the Just a Face and a Caption page under True JAFAAC: "A caption may be inside the image or even disguised as something else, like a Speech Balloon".
The horse's design in contrast to the rest of the picture illustrates how it's not natural to the setting, with Gabe's text clarifying what it is. It's weak, yes, but it's not a bad or harmful image like the Mona Lisa one and it's not JAFAAC.

The important part of a DLC image is that it illustrates that the content is purchased separately from the rest of the game.
Personally, I'm ambivalent. The image really doesn't illustrate anything, but I don't know if I'd consider it harmful either. That said, the crowner seems to be in favor of not having an image at the moment.
