Main page. Given that this has been a "no pic" trope for a long time now, I think we can make an exception to the "pics go on subpages" guideline in this case - put it on the main page and have a "no pics - see the main page for a depiction" notice on the subpages.
Hum. I don't think that is a good reason for putting medium-specific images on the main article.

I think it's a good idea to put up any image on the main page if none of the subpages have an image.

Normally I would agree, but this (and any of the other "no pic" pages that may be subpaged) is an exceptional circumstance.
18 works for main.

I think 31 is well known enough to be seen as amusing rather than prurient so I'm switching my vote to it too.

As far as aesthetics go, I personally would prefer 31. It's not really any better or worse in terms of showing the trope.
I like the suggested caption in 34.
