Well probably the same as anyone else torturing someone actually, except they would be less likely to overdue it killing the patient. Humans have done it for thousands of years so they kind of have it down.

How would a doctor torture someone? Well, not even a doctor, really. Just someone with extensive knowledge of the human body and access to various medical tools and pharmaceuticals. Please include anything that comes to mind (this includes naturopathic medicine like acupuncture and herbalism).
Additional context: the "patient" is to be kept alive as long as possible, and any non-fatal injuries are allowed as long as they don't cause complications or conflict with the first demand.
