I wouldn't know about this if it weren't for obscure gaming companies self-referencing themselves, as was the case of making Time Gal's protagonist playable in the third Castle Shikigami game. Where she's actually pretty awesome.

The animation and her character design look extremely familiar, but I can't quite remember where I've seen it from.

I love how getting her shirt torn is as bad as getting blown up.

So I found this somewhat funny animeish Dragons Lair sort of game on Youtube, and it's actually somewhat fun to watch, if partly because some of the failures are so silly(though a few are pretty clever, like the caveman one). It's basically one enormous QTE but the animation is pretty good despite some reuses. It was for arcades originally but was released on the Sega CD later on.
Basically, a dude in the far future steals some time beam weapon and Time Gal has to stop him by travelling through time, each era/level has a few QTE sequences which has Gal dodging/shooting stuff and occasionally she does a time stop which gives you three choices to pick from. Pick the wrong one and she dies/loses(complete with the villain laughing at your demise), pick the right one and you progress forward. It's pretty simple but somewhat fun to watch, also she seems to have a lot of fun doing all of this. Some of the failures are pretty funny.
Failure collection here.
