There's a random thing that I've been wondering. Why do people call Tywin Lord Tywin? Instead of Lord Lannister? Every family patriarch is called Lord (Surname). Lord Stark, Lord Baelish, Lord Tyrell, Lord Bolton. Why is Tywin an exception?

Maybe Tywin's pride? The Lord Lannister before him was a joke and maybe when he came into his Lordship, his vassals still sniggered at the name Lord Lannister, so Tywin possibly wanted to differentiate himself from his father's failures. That's my guess.
We know that Ladies are called either (Lady Catelyn, Lady Stark, Lady Olenna)

He's trying to get "Lord Lannister" into the history books as one of the most powerful and feared men in the Seven Kingdoms.
It's probably just a show thing to make it clear which Lannister they mean but it may be that in Westeros they use "Lord Firstname" in a more conversational manner while "Lord Surname" is a bit more officious.
That just leaves me wondering who the Hell speaks with Tywin in "a conversational manner". Everyone speaks to him like he's Satan (including his children), I don't think most people would have the balls to call him "Lord Tywin".
"All you Fascists bound to lose."From what I can tell, the rule for titles seems to be that the person in charge is Lord Firstname Lastname and their wife is Lady Lastname (although women will sometimes go by their maiden names). Children don't have a title, but as a courtesy will sometimes be called Lord/Lady Firstname.
So you have Lord Eddard Stark married to Catelyn who is called variously Catelyn Stark/Catelyn Tully/Lady Stark, and their daughters are occasionally called Lady Sansa or Lady Arya.
Edit, edit, edit, edit the wikiWe see most every character refer to Tywin as Lord Tywin or Tywin Lannister. Directly in his presence, speaking about him, or thinking about him.
I don't think it's a rule. People say Lord Stark, Stark, Lord Eddard, Ned Stark, or Ned depending on the circumstances and their familiarity with him. And one great "dear old dead Ned"

Trump delenda estMerely headcanon, but given how Tywin was already formidable when his father was Lord Lannister, it wouldn't surprise me if people called him "Lord Tywin" as a matter of course from adolescence, and it just stuck.

House stark: Mostly wiped out, with it's heir scattered all over, and only one of them still calling himself a Stark (the other being a bastard).
House Arryn: Lost its head (both the Patriarch and the Matriarch), with the only heir kinda being dumb.
House Lannister: No heirs (Jamie is still King's Guard, Tyrion was out of the running even before he became a Kin-slayer, and Cersei is a woman), and its head just had an intimate meeting with some crossbow bolts.
House Tagareyen(sp): It's only current heir can no longer have children.
House Greyjoy: Only male heir is the current bitch of the Boltons.
Simply put, a large number of Noble houses have been wiped out like the dirt on our shoes.
The Baratheons still have Stannis, the Frey's have an army of heirs, the Boltons still seem good, and House Bronn is on the rise, but a lot of the old school guys have been buried like the family dog.
There are some details I obviously can't mention due to being from the book, but a lot of noble families have been wiped out.

Stannis Baratheon is the only one with family still more or less intact, which was the argument used for them to get a loan from the iron bank. Despite losing the battle, he's still in his prime and the rest of the houses are falling apart.
That's said the Lannisters' still have Toman, who unlike his older brother and mother is not a total psychopath so I wouldn't count them out yet.
House Martell of Dorne is okay, they wisely only sent their bastard prince to Kingslanding.
House Tyrell, is still quietly Growing Strong

hashtagsarestupidTommen, in the eyes of the law and according to the Lannisters, is a Baratheon (and probably, if the Lannister's win, the leader of the Stormland's and Lord of Storm's End). If the Lannister's mean to keep Lannister's in Casterly Rock, they'd have to turn to cousins (like Lancel). Question, do we know the Lannister family tree in the show? Like how many brothers Twin has or Joanna's family?
What? But that would require him playing a role not in the friend zone!
