Alright, here's the deal. I have a character who's just got to die so as to drive home a point. But i don't want them to just die doing something heroic. I want to watch this person burn themselves out for full dramatic effect. Ideally, I'm looking for something that will leave them looking healthy as long as they take things easy, while slowly and subtly eroding their health. But that is also exacerbated by physical exertion to the point that it could turn years of relaxed living into a few short months of near constant action. I'm not all too picky as to how this happens, genetics, illness, addiction... I'm just looking for interesting options and have to admit a minimalist understanding of medicine.

Need to know about strange weapons, especially weird guns? I know em, and if i don't I'll find them.

What's the level of medical progress in the setting?

Also, how realistic is the setting? Because if your work is fantasy or sci-fi or something similar you could always just make something up.

Even if it wasn't you could make something up provided it sounded suitably realistic.

Seriously. Influenza would do it.

It's beautiful and so full of deep imagery that it doesn't surprise me to find that it has gone WAY over your head

You could use tuberculosis for the whole Incurable Cough of Death effect. It's got the degenerative quality, but I'm pretty sure lots of exertion (and hence strain on the lungs) will exacerbate it.

The Incurable Cough of Death is vastly overused - I'd advise against featuring that as a symptom.
