The way the old twins' pose mirrors the pose of the twins in the picture, it seems pretty clear to me they're supposed to be the same persons. I have no trouble understanding the pic.

It's pretty obvious what the trope is about. I don't think it's a very good image, though, mainly because it's hardly clear that the old twins in the foreground are actually the same as the young twins in the backgrounds. Aside from that, what's with the twins? Or perhaps it's just a reflected image for no reason. It distracts from what the trope is about. Finally, it's rather low-quality art.
I'm sure we can do better than this.
