Ursula is not an example according the to the trope page itself, except in Re:CoM. We should probably go with something else. King Kaliente (the Super Mario Galaxy boss you mentioned but forgot the name of... at least, I think that was his name; there definitely is a boss with that name in there) is a textbook example; I think that's the best proposed option so far.

For some reason, the fights against Ultros in Final Fantasy VI come to mind. When he attacked you, it showed a tentacle smacking the character. I wish I could find a screenshot of that specific action.
