Okay, I've put a cropped version of the image up on the page. I've kept the original caption for now, but should we perhaps use it for the crediting requested in the second of the artist's conditions? The image currently links to the site requested, although that places a space between the image and the caption - if we place the credit in the caption it could be made a hyperlink to the site, satisfying conditions two and three.
My main hesitation is that, given the length of the current caption (with the hottip included, at least), I'd be inclined to either shorten it or replace it entirely with the credit.

Cool. I think we're pretty much done here, but I'm gonna set a new clock and wait until we hear from the artist for confirmation of the permission or it runs out, whichever comes first.
EDIT: Made a few small tweaks to the hottip; putting quotation marks inside links is kinda frowned upon.

I just heard back with regards to my note to the artist; the relevant section follows:
"Thanks for the note. It would be great if you could insert the line "Illustration: John Howe" and set up the link to the site page of the image itself. That way, the copyright info and all will be clearly visible at one click's remove."
I've made the changes requested, I believe. I would have preferred to have had the attribution on a new line in the caption, given the long hottip, but didn't find a way to do so.
